Suggest Remedy For Severe Pain Caused By Avascular Necrosis In Pelvis

Question: Hello..I have recently been diagnosed with bilateral avascular necrosis in pelvis..much of it has apparently deteriorated already. It has also spread to my knees and sometimes my feet get numb. I had a bad fall in May 2014, thats when this started and I was told until a month ago that it was "only" osteoarthritis. I am in constant pain even at night. I haven't been able to get upstairs for 2 mons, stairs are excruciating & I can't bend at the waist ti pick up anything. I use a walker to get to the main flr washroom and cannot stand on my own w/o it for more than a couple minutes. I am on oxyneo20mg and tridural 300mg for pain and also toradol..it only helps a bit. I can't get to work, and am also starting to have incontinence. I can't believe in a year what this has done to my life. I've been told here in Alberta it will be 6-18 months to even get a hip replacement..honestly I don't think I can take this pain that long. I am 54 yo, weigh 150 lbs, am 5'3", my aunt had arthritis but it was nothing like this. It feels when I can stand like my leg bones will break and they make horrible cracking sounds..can this happen..can this be fatal? Is there any surgery that will help? How much worse can this get?

Do you find your back to be very stuff that you can't bend forward?

Any difficulty in vision?

You seem to be having severe arthritis of both the hip joints. This classically causes referred pain to the knees which is why your also have knee issues.

Hip replacement is the best bet you have. Once the hips are replaced you will have better mobility and tolerance of both your hips.

I want to rule out Ankylosing spondylitis, which can also cause stiff spine. Kindly upload your X-ray of your Spine and hips if possible.

The pain shall subside once the hips are replaced. Not to worry.

I have been hearing various things about the success of hip replacements with this disease..sometimes they help but not for everyone. As i asked, as the pelvis deteriorates, can it fracture.. it sure feels as if it could and same with my leg bones.
Also, what is Ankylosing Spondylitis?
and what else can I take for pain control?

Hip replacement is required in severe hip arthritis

Detailed Answer:
Usually there is no other go than hip replacement in the setting of severe hip arthritis.

Ankylosing spondylitis is an inflammatory disorder affecting the spine and hips causing stiffness and arthritis.

An Xray of the spine should be enough to make a diagnosis. It would be best to discuss with your treating surgeon about the benefits and risks of Joint replacement in your case.

Since you are already on strong pain killers, I can only suggest you take intramuscular pain killer like Diclofenac 1 ampoule once during a severe pain attack. This needs a prescription and needs to be administered by a trained health professional. The risks include pain in the region of injection, possible infection, possible nerve damage if administered in wrongly and also possible hypersensitivity reactions. Hence, please keep in mind the risks and kindly discuss with your Doctor for the same. The better methods of pain relief will be reduced weight bearing, use of walking aids. Kindly avoid Unprotected weight bearing or rather perform protected weight bearing with Walker, crutches or walking stick which will also help in the pain relief. Apply warm or cold packs, whichever relieves your pain.
