Suggest Remedy For Abscess In Anus While Having Crohn's Disease

Question: I have abscess near anal area (close to anus). The lump is quite hard and I can feel that. Also I have Crohn's disease (IBD) for which my treatment is still going on.

I have already consulted a general physician (MD) and took course of antibiotics for 7 days.
But once the course is over the lump appeared again.
While taking the antibiotics my IBD problem also increased as I had repeated loose stool.
When I consulted my Gastroenterologist he suggested to stop taking antibiotics but did refuse to treat the abscess. He told me to revisit the same physician I consulted earlier.

1. I want to know what is the best treatment for this type of abscess which at the same time not cause problem for my IBD?
2. What type of doctor is best to treat this?
3. Should I consult a dermatologist for this? Does a dermatologist treat such problems?
4. Or should I consult any new MD or a Surgeon or a different Gastroenterologist?

A seven day course of antibiotic would have totally controlled the infection.
There are chances that due to IBD absorption of antibiotics might have been lower.
You might require treatment with injectable antibiotics.
There might be requirement to drain the abscess.
Better meet a general surgeon and discuss regarding this.

Hi

Thank you for the response. Please suggest,

1. Whether it's fine to continue hot water compression with cotton or any similar activity at home to ease the swelling?

2. Before going to a general surgeon, if I consult another general medicine doctor for opinion but this time at a hospital OPD will that be of any help?

After diagnosis if he suggests that drainage of the abscess is required, then I will meet a general surgeon.

Kindly advise in reply.

Yes hot water compression will be a good idea.
I feel it's better to meet a general surgeon directly.
Any other doctor might put you on antibiotics again without draining it and the problem might persist.
Hence it's better to meet a general surgeon directly to save time and money.

Hi

Thank you for the advise. As per your suggestion I visited a general surgeon today. After diagnosis he said my abscess has turned into fistula.
He said drainage is not required as of now. He advised dressing with Betadine lotion and Bifilac tablet. However he also said there may be requirement to operate the fistula.

Now lastly I want to know,

1a. What is the process of dressing with betadine lotion , shall I use hot water or normal water ?

1b. Is bifilac tablet harmful for GI Ulcer and IBD patient ?

2. What is best treatment for this type of fistula ? Or is operation the only solution ?

3. Is it a major operation for which I might need to take admission to hospital for a week or so ?

4. How soon a patient can recover from such operation ?

5. Even after operation will it be cured completely or there is any chance of recurrence ?

6. What type of care should I take so that abscess or fistula does not appear again and my GI Ulcer also gets cured ?

Please advise in reply.

Thanks for the follow up.
The definitive treatment for fistula is surgical.
It a permanent cure and fistula usually doesn't re-occur.
Bifallic is safe for GI tract.
Soak betadine in a gauge and apply it over the fistula and let it be there for 10 min.
You can wash with warm water and then apply betadine.
Take probiotics daily, this will keep your GI tract healthy.
