Brief Answer:
Most probably colitis.

Detailed Answer:
Hi.
Thanks for your query.
Read and understood your history of pain in abdomen as explained by you, left - jabbing high up under ribs - cramp through left and right lower abdomen - referred pain to groin and mid-back pain; changes in stool and urine. Also noted your history of visit to GP and Physio.

As per the history you have provided the problem looks more of colitis.
I would advise you the following:
Stool tests for 3 consecutive days.
Test of urine: routine, microscopy, for bile salts and pigments.
Routine tests of blood.
A 5-day course of an antibiotic, probiotic to see the effects (need a prescription from your GP and his thought of giving it or the stool showing up positive for infection).
Rest of the investigations and treatment as per the clinical evaluation and reports and response to the treatment.
Reference for a Gastroenterologist.
Further tests that may be needed are colonoscopy and CT scan of the abdomen if required.
Your GP might have thought of the back nerve causing abdominal problems, but in such a situation the urine and stool will be normal, hence the differential diagnosis of colitis and so on.
