Question: I have painful urination and my bladder doesn't seem to empty, I have frequent panic urination with very little urine coming out. This has been coming and going for a few months. When I go to bed this seems to stop and the pain seems to leave until the next morning and then is starts again. I have accidents when I can't make it to the bathroom in time.

To recapitulate: Male/65 - painful urination - accidents where you can not make to bathroom - frequent panic urination - small quantity - few months - okay in bed - took antibiotics - stopped Plavix to see it helps - noted the medications...
Considering the age I would think as prostatic hyperplasia as the commonest cause to think about.
I would advise you the following in such a situation:
Urine: routine, microscopy, culture and sensitivity.
Tests of blood: CBC, PSA, Kidney functions and as needed.
Ultrasonography of the whole abdomen, particularly of genitourinary system - for prostate in specific.
Clinical examination, per-rectal digital examination for prostate, preferably by an Urologist.
These will give the exact cause of the symptoms and a plan for further management.
