I am Dr.Ajay Panwar,a neurologist,here to answer your query.

Probably,you are talking about Periodic limb movements in sleep also called as Nocturnal myoclonus.
If yes,medicines used are mainly of the following classes:
1)Dopaminergic drugs-Pramipexole and Ropinirole
2)Benzodiazepines-Clonazepam and Temazepam
3)Anticonvulsants-Gabapentin and Pregabalin
4)Opioids-Codeine,oxycodone,hydrocodone,methadone

Your medical history mentions that you are on a combination of medicines belonging to all the above classes.Along with medicines,non-pharmacological measures are also important,which are-
1)Sleep hygiene
2)Avoiding coffee,tea,smoking,alcohol
3)Mild exercise regularly
4)Hot bath 2-3 hours before bedtime
5)Remaining mentally engaged

There is nothing like difference between myoclonic jerks and PLMD.There are several causes of myoclonic jerks,out of which PLMD is the one.
PLMD is characterized by myoclonic jerks during sleep(nocturnal myoclonus) which appear at regular periodic intervals.This can be taken as the hallmark feature of PLMD.Other causes may have myoclonus at daytime(while awake),not necessarily with regular periodicity.
