Along with history, MRI aids in diagnosis.

I am Dr.Ajay Panwar,a neurologist, here to answer your query.
Yes, MRI of brain can help in diagnosis by showing vascular changes or multiple evidence of multiple ischemic insults. However, it is only in agreement with clinical history and examination that the diagnosis is made.
Hope that helps. If you have some follow-up questions,I shall be glad to have you in follow-up.
Yes, misdiagnosis is a possibility.
Thanks for being in follow-up.
Of course, if the symptoms have improved completely, then misdiagnosis may be a possibility. MRI can't confirm the diagnosis of LBD. It can be just supportive and more important is a historical and examination based diagnosis of Dementia. SPECT, in fact is a better tool for diagnosis. Besides, if the symptoms have improved vastly, then I would say no imaging is superior to the clinical presentation, especially in case of Dementia.
Hope that helps. If you have any further questions,I shall be glad to have you in follow-up.
