Brief Answer:
further necessary information before management

Detailed Answer:
Dear
Thanks for contacting with your health query and after reviewing your case history I'm of the following opinion:

1. As you are suffering from XXXXXXX syndrome [inherited gene mutation] causing constitutional hepatic dysfunction thus there will always be a slightly yellowish color of eyes [bulbar conjuctivae] but it is not dangerous.

2. Thalassemia minor [inherited blood disorder]: As it does not pose any serious medical disorder thus also not too dangerous, however the person due to slight defect in hemoglobin synthesis [resulting in small size of the red blood cells] the person will remain slightly anemic thus do not go for any medicine which will increase iron content in the body as it will result in iron toxicity and iron chelation therapy is part of treatment

PS.
i. It is important that you screen your children for the same to help identify any concomitant syndrome or trait in them.
ii. since the presence of both conditions are associated with increased prevalence of having gallbladder stones check with same and remain under the regular supervision of a Hematologist

3. Although IBS is a common symptom in people having XXXXXXX syndrome make sure to have an appointment with a Gastroenterologist to exclude any other cause of IBS [stress/infection/genetic]

4. Since in prescribing medicine one has to take extra care regarding liver, spleen and not to increase the iron content in the blood thus I do prepare a combination of medicines and liquid which helps overcome both the conditions and at the same time doesn't elevate the iron content, if interested I can courier you the medicines since taking into account the three associated condition no single remedy or medicine will be effective in you.

Brief Answer:
Hepatosplenomegaly and other information

Detailed Answer:
Dear
Thanks for reverting back

1. As you are suffering from thalassemia minor which is a group of inherited microcytic, hemolytic anemia characterized by defective hemoglobin synthesis thus due to hemolysis there is spleen enlargement [since red blood cells are destroyed in the spleen] and slight liver enlargement could be due to some associated liver cause or secondary disease
PS. consult with a Hematologist or Medicine specialist to exclude other common causes of hepatosplenomegaly [as the list is exhaustive]

2. As the colonoscopy report reveals only ‘patchy edema’ and nothing else thus it could be due to frequent passage of diarrhea resulting in mucosal injury and edema and as history of stress is also there thus people with IBS are more sensitive to emotional stress since it makes the mind more aware of spasms in the colon
PS. consult with a General Surgeon to exclude any other cause of anorectal disorder since the history of alternate diarrhea with constipation is also there. [could be laxity of sphincter muscles]

3. Till you get an opinion from the specialist I will provide you treatment and management on the basis of ‘FUNCTIONAL BOWEL DISORDER’ [GRAHANI]
PS. as symptoms increases/worsens during time of stress: thus probably could be Hypersensitivity of Intestine [Functional Bowel Disorder]

4. Dietary Regimen

i. Take: old brown rice, moong/masoor/arhar dal, banana, jamun, buttermilk, goat milk, sesame oil, honey, soup, cumin, mint, fasting.

ii. Avoid: newly harvested rice, peas, garlic, vinegar, milk, jaggery, citrus food, spices, nighttime awakening, drinking excess water, suppression of natural urges, excess physical exertion, walking during peak hours in summer.

5. HOME REMEDIES

- make buttermilk [lassi], every day and drink, make sure it is made/prepared fresh every day, do not add water, add fried cumin seeds and rock salt [quantity sufficient]

- fried cumin seeds 500mg
fried asafoetida [hinga] 250mg .. take with honey, thrice a day.

- soak 5 gm amla in water, when becoming soft, crush them and add black salt [quantity sufficient], and make lozenges of 125 mg and suck twice daily.

- hot hip bath: sit in a tub of warm water for 10 minutes [2 times a day]
PS. Warm baths can relax the anal muscles, relieve irritation, and increase blood flow to the area. [initial ano rectal procedure]

6. Yes, there are medicine(s) which are prepared by myself in such cases and which are result oriented and effective too [evidence based medicine], I can courier you the same if interested to overcome your pathology since IBS is a psychosomatic disorder thus will cover both the physical as well as neurological aspect and also will take care of the hepatosplenomegaly.

hi sir thanks for reply...
I have some comments as per Ur point numbers.
Also I missed something previously to tell you that whenever I stand from any position I just get almost choked, blackness on eyes and it's for nearly 10 seconds everytime. it's been 4 months now.
1) does spleenomegaly playing any role in my fatigue.
2) the patchy edema is due to the pressure I used to contract my rectum eveytime for one yr and used to squeeze my rectum many times. this is near tail bone. this patchy edema has just become my habit now -effortless habit. I now need to relax this to pass stools which is very difficult. That's why I was asking you some therapy (massage or something) in ayurveda to relax rectum.
I have consulted my doc there is no prob in sphincter.
4) taking best diet- buttermilk and all.
6)which medicines- is it for blood or for rectum. how much tym will they take to impact. any side effects?
Shall I send u my all reports on Ur mail so that u can c it better.
thanks

Brief Answer:
further clarification

Detailed Answer:
Dear
Thanks for reverting back

1. Cause of dizziness/lightheadedness/choking sensation on standing up [at your age] could be due decrease in blood volume or autonomic nervous system dysfunction so a Medicine specialist opinion is necessary [for an underactive adrenal gland or POTS (postural orthostatic tachycardia syndrome)]

2. Fatigue is not a symptom but an underlying cause of spleen enlargement thus your existing pathology [thalassemia minor] is contributing towards Hemolysis which is causing:
- Increased red blood cells death causing FATIGUE and
- Spleen is the main site of destruction for the abnormal red blood cells thus causing SPLENOMEGALY

3. Regarding relaxation of rectum initially, start with the sitz bath [warm hip bath] as described earlier and later one can ask for Vasti [enema] therapy in Ayurveda

4. The medicine will be acting both on the blood [spleen] as well as the rectum and you will notice improvement/general well being after 2 months [some have reported earlier] and no side effects have been reported or documented [till date]

5. If you wish to continue further you may send the reports but your case history has provided me with enough clarifications.
