Question: My wife is 69 years old, 5' 4" suffering from malnutrition and her weight has dropped to 103 lb.
She has interstitial cystitis, irritable bowel syndrome, fibromyalgia, lactose intolerance, gluten intolerance, and chronic (occasionally acute) pancreatitis.
What would you recommend for her to maintain optimal nutrition and gain weight?

Hello! Thanks for putting your query in HCM. I am a Gastroenterologist (DM). I appreciate your concern regarding your wife. I will describe diet for all disease one by one

Interstitial cystitis and fibromyalgia- no special diet

Lactose intolerance and gluten intolerance obviously you have to avoid milk and its products and all the gluten containing meals respectively

For chronic pancreatitis take high protein, high caloric diet. If She is having diarrhoea then She should avoid high fat diet. then fat content should not be more that 20gm/ day. Instead She can take medium chain triglyceride present in high content in coconut oil

For irritable bowel syndrome avoid beans, cabbage, milk, rice, and pulses. Take high fibre diet

In conclusion high caloric, high protein but low in fat and rich in fibres will be the diet recommended for her. You may also take the opinion of a dietician to make diet chart for her according to her prefference or taste
