So I’m A Little Confused As To What’s Going On

Question: So I’m a little confused as to what’s going on with my body. I took femara this cycle but never got a positive opk. I had spotting cd 17-19 and cd 27-28. My dr gave me provera to take starting day 28 since I hadn’t ovulated & had a progesterone done that was 0.1. I took one provera on day 28 then woke up on day 29 and I had a positive opk. I didnt take anymore provera since I got the positive & assumed I was ovulating. Woke up on cd 32 with cramps, back pain, and spotting again...same today with cd 33. The cramps and back pain feel exactly like it usually does when I’m getting my period but there is only enough blood to wear a liner. Idk what to do now. I’m just confused by all the spotting. Should I just take provera now anyway and get my cycle restarted? Or do I try to wait it out and see if this turns into a full flow.
I’m due to start my femera again on cd3

Some feedback. Repeat urine pregnancy test.

Detailed Answer:
Hi there
I welcome you to HCM virtual clinic.
I have understood your concern and I will suggest the best possible treatment options.

Please do not panic.

I would like to know about few things in detail

What was your LMP date?
On which days did you start taking your treatment?
Did you get such kind of spotting just before expected date of period anytime in the past?
Have you undergone ovulation study by USG or on urine test kit?

Secondly, please get morning's first sample of urine tested for pregnancy.
If it is XXXXXXX positive, then please consult with your treating doctor.

Please feel free to communicate with me again.
I hope this answer helps you.
Thanks.

My lmp was dec 26. I took femara from cd 3-7. This was the second cycle on it. The first cycle was 2.5mg and ovulated on cd27. Ovulation confirmed with progesterone 7 days after with a result of 9.0. This cycle I took 5mgs. I had a progesterone done on cd 27 and it was 0.1. I’m now on cd 33. I track ovulation by using the clearblue digital tests. I’ve never experienced any type of spotting during a cycle before. I had a trasvaginal us done on cd 6 of this cycle and I will attach the report. I also took a pregnancy test this morning but it was negative. The spotting is now a little heavier with left flank pain.

Brief Answer:
Lifestyle modifications, ovulation study

Detailed Answer:
Hi there
Thanks for appreciating the answer and reverting back to me.

I have seen your reports.
USG report is suggesting PCOS- polycystic ovaries.
In such situations I usually suggest lifestyle modifications, along with medical treatment.
Please opt for a healthy diet and regular exercise regime.
Include more portions of fruits and vegetables and salads in the diet.
Avoid refined sugar and deep fried foods and bakery products.
Drink plenty of water.

Also , I will suggest you to get follicle study from day ten onwards on alternate days along with ovulation promoting medicines.
Plan to have unprotected intercourse on and around the day of ovulation or IUI - Intrauterine insemination on the day of ovulation.

I hope this answer helps you.
Please feel free to communicate with me again.
Thanks.
