Question: Sir, I am 45 year old male, weighing 91 kg, with history of hypertension. I am presently using Telemikind-H 40, once a day. Three years ago, I was diagnosed with Gilbert's Syndrome. Please find enclosed my latest blood work and urine analysis report. Two years ago my serum creatinine level was 0.9 and now it is showing 1.1. Do the numbers indicate any problem with my kidneys. 4 months ago I underwent ultrasound scan of abdomen, and everything was found to be normal. Kindly let me know the suitable course of action.

Thank you, Sir. I use Telemikind-H 40 mg, which contains Hydrochlorothiazide 12.5 mg for hypertension (once a day). I have been using it for nearly 5 years. Does the long-term use of diuretic harm your kidneys. Should I use any alternative drug? If so, can u suggest me an alternative drug.

Telmisartan and Hydrochlorthiazide (40+12.5mg) is a very well tolerated and safe medicine combination for treating hypertension.
There will be some or the other side effect from every medicine, particularly when taken in a way not recommended by your treating doctor.
All people do not develop all known side effects of any given medication.
There is a long list of side effects associated with hydrochlorthiazide, but only some people develop some of these side effects.
Not all side effects are serious too, most of the side effects of hydrochlorthiazide are very mild and do not warrant any change of medicine.
The fact that you have been taking this combination of Telmikind-H 40 for 5 years means that you are not having any side effects from this medicine.
Long term use of this medicine may harm kidneys in some people only, but that kind of risk will be present for almost all anti hypertensive medicines, including telmisartan and all other medicines that are used to treat high blood pressure.
The doctor always considers the potential risks of using a medicine vs benefits of using the medicine, when benefits outwiegh the risks, doctor will ask you to take the medicines.
In your case, as you already taking this medicine for the last 5 years and no side effects have occured so far means it is very safe in for you and benefits of continuing the medicine are much more important than potential risks.
