Question: My girlfriend got bitten by a insect or bug of a 17th floor of a hotel terrace in Phnom Penh Cambodia. She is very tolerant to pain. But she screamed and began to cry at the moment of the bite because of incredible pain. So I brought her to the nearest hospital. They gave her some antialergetic medicine cleaned the area put fenistil cream on it. But after couple of hours the pain still contuniues as the same as the beginning. The area get smaller after couple of hours but the pain is still same as the beginning and its terrible. She is very very tolerant to pain. But this is something she couldnt handle. It seems no other reaction happened on other parts of her body. But the area where she got bitten(her back) her right arm and right chest are in constant pain, even after 2 hours of parasetemol serume. We couldn’t catch the bug or insect so we don’t know wohat it is. We need instant help, thank you.

Brief Answer:
Give her higher painkillers.

Detailed Answer:
Thanks for your question on Ask a doctor forum.
I can understand your concern.
Some insect bites can cause intense local site inflammation and nerve damage.
So amount of pain will be very high.
Paracetamol is the basic anti inflammatory drug.
It's painkiller property is not that good.
So she should take higher painkiller drugs like ibuprofen or acetaminophen or centrally acting (acts on brain) painkiller like Tramadol.
Pregabalin or Gabapentin should also be started if it is nerve related pain.
Application of ice packs on affected areas will also be helpful.
It would be of great help if you can send me picture of bite.
I will be happy to help you further. Wishing good health to your girlfriend. Thanks.
