Question: My daughter is about 1 year. She doesn't eat food . We are feeding her too forcibly and even though she is hungry she will ask for breastmilk but won't eat any fruits or cereal. So she is underweight. Any suggestions to make her hungry and eat something by herself

Hi XXXXXXX
Welcome to Ask A Doctor service
I have gone through your query and understand your concern

This is a common issue that we face, but most of the times, is not something to be over-worried.

In order to have a more clear picture, kindly give me few more details:

1. Her birth weight?
2. Her present weight, and if possible, length/height too
3. What was her weight 2-3 months back?
4. How is her bowel habit- any constipation?

Kindly get back with details, so that I can guide you better

Brief Answer:
Treat constipation, rule out anemia

Detailed Answer:
Hi XXXXXXX
Thank you for the details given
Sorry for being late, as I was very busy today

I could not understand her height- you mentioned it as 7.4, but unit not mentioned(it can not be 7.4 feet/meter)

Anyhow, her weight is inadequate. By 1 year, we expect around 9 kg. So it is 86% of expected weight. Although anything above 85% is acceptable, here she has not gained much in last 2-3 months. We expect around 400g gain per month, but she has gained only 400g in 2-3 months.

But we need not worry. We can improve with few simple measures:

1. I guess she do not get frequent issues like respiratory infections or loose stools. If there is, we have to tackle this for proper growth. Complete all immunisations as per schedule

2. We need to rule out anemia by doing a blood count(CBC+ESR). Anemia will not only cause poor appetite but also will prevent proper assimilation of nutrients. If detected, we can correct this with iron supplementation

3. She is suffering from constipation- this need to be corrected to promote feeding. For this, include lot of fibre rich items in food(green leafy vegetables, oats, fruits) and make her drink enough fluids.
If these measures are not helpful after 2 weeks, I can give you a laxative then.

4.Now regarding feeds: Give feeds in frequent small amounts, rather than as 3 major meals. Do not force feed once she warns to stop. Add oil/ghee while preparing food- this increases calorie density. All meals should contain a protein source(pulses/dairy/fish/meat/egg).
Do not restrict breast milk- give as and when needed. Empty one breast before changing to the other. If you are giving cow milk, give undiluted with added sugar.
Avoid tea/coffee as far as possible. Promote thick rice water instead of plain water to quench thirst.

5. Deworm her by giving Syrup Albendazole(200mg/5ml) 5ml if not yet given

With the above measures, definitely she will improve in 1 month. Otherwise, we will evaluate further

I hope this answer will help
