Question: Is there any disease process or issue related to your lumbar or sacral spine that could cause your internal anal sphincter to not open or operate properly thus leading to a buildup of gas and the inability to pass gas even though you feel you need to?

I have noted your clinical details. These clinical symptoms can not be explained by any lumbar or sacral spine disease.

If there is involvement of lumbar or sacral spine/nerves, there would be involvement of all these three- bowel, bladder and sexual functions. This is because the nerve supply for all these three functions come from sacral nerves (S2, S3 and S4). Bowel symptoms are usually constipation. Bladder symptoms can be retention of urine or urinary incompetence. Sexual dysfunction usually manifests as erectile dysfunction.

Then i guess i need ideas....i get really gassy..i think because i have rapid gastric emptying as shown on my small bowel follow through. What would cause you to not be able to pass gas...even though it is dispersed throughout your colon and through your large intestine.

As a neurologist, I can say that you do not seem to have a neurological disorder.

You would require a detailed gastroenterological evaluation, which could include endoscopy, especially colonoscopy, CT or ultrasound of the abdomen, etc. After that, a specific diagnosis can be made.
