Question: Is prednisone often used to treat ongoing kidney weakening with skin itching, some anemia and really terrible fatigue, and inability to concentrate?

Is an estimated GFR of 28 low enough to consider long term low dose prednisalone treatment. I think I feel a lot worse than the score shows and I really need to feel better to keep functioning OK. Thanks!

Brief Answer: Prednisolone alone can not help. Detailed Answer: hi, eGFR <30 is considered as stage 3 kidney disease. There are lot of drugs that can improve eGFR even at this stage, like cyclophosphamide etc. Prednisolone alone can not effectively tackle the lupus kidney at this stage, as here protein leak is not the main problem. I know how it feels like suffering from lupus, as it is a multi systemic disease, eGFR alone can not explain symptoms. You may need monthly injections of cyclophosphamide, which surely will help your kidneys. For this, a Nephrologist consultation is required. hope i am clear.

You are very clear and very helpful and I am most appreciative! I will definitely get a referral from my GP to a nephrologist. According to my rheumatologist, my lupus is inactive with a normal ANA right now, which is why I figured my current problems must be kidney related. And there's the cardiologist, also. When you are kind of old and feeling lousy, it's pretty hard to keep all three doctors contributing pieces from their fields to contribute to my over-all efforts to feel better. Your are helping me to see the possibilities. I think I'll work first on nephrologist to see what help comes from there. And leave the lupus in peace for now. Do you agree?

Brief Answer: Absolutely right. Detailed Answer: Absolutely right. Being someone who is in-charge of kidney patients for 6 years, i can confidently tell that the treatment for lupus nephritis not only helps kidneys, but also lupus. You will definitely improve and will hopefully mail me back with much better eGFR results after consulting a (good) nephrologist. I hope for it!!!!
