Is Pelvic Floor Weakness Due To The Hysterectomy?

Question: 63 yr old female with Lumbar laminectomy for spinal stenosis and disectomy L3-4, L4-5 due to rupture and obliteration 12 weeks ago,. No pain following surgery. Starting week five, started feeling pelvic pressure with BM or urination before and after. I had total hysterectomy 14 months ago and breast cancer 2008. Went to GYN this week who ruled out UTI and pelvic floor weakness. Is this a result of the surgery?

Unlikely to be due to surgery; UTI is a common cause

Detailed Answer:

Hello,

If a complication occurs due to surgery then in most cases it occurs immediately after the surgery. Its unlikely to be due to a delay of 4-5 weeks (or 14 months in case of hysterectomy). Urinary tract infection is a common cause of fullness in pelvic floor along with the symptoms you are sharing. If i were your treating doctor then i would have advised following measures:
- Drink adequate water.
- Take very good care of personal hygiene.
- Wash genital area thoroughly after using washroom.
- Avoid coffee, alcohol, and soft drinks containing citrus juices and caffeine
- Wipe from front to back. Doing so after urinating and after a bowel movement helps prevent bacteria in the anal region from spreading to the vagina and urethra.
- Get an ultrasound of abdomen & pelvis done.

Hopefully you will see improvement in 2-3 days but if condition remains same or aggravates then you need to visit hospital for further workup.

UTI and pelvic floor weakness was ruled out with a Gyn visit this week, as I included in my question. She did not feel any pelvic masses either. Any other thoughts?

Muscular Spasm could be the other cause

Hello

Thank you for getting back with the answers. Other common cause include bowel disturbance followed by muscular spasm. Post-surgucal mannupulation (although unlikely after this period) may also be the cause as common causes have been excluded. Following mesaures will hopefully help n improving the status:
- Eat high fiber diet
- Aoid carbonates drinks
- Less carbohydrates.
- take Phloroglucinol + Trimethyl Phloroglucinol (generic name Tab Spasfon) antisposmadic medicine once a day for 2 days.
