Question: Hello i just seen paediatrician surgeon for my son he is 7 years old and she said he has balantitis xerotica obliterans she said that circumcision is needed. we not very happy to do full circumcision and was wondering if preputoplasty is something we could try first as we feels like circumcision is the last option if we can do anything else.

Brief Answer:
I would prefer preputioplasty

Detailed Answer:
Hello

Thanks for the query

Circumcision remains the main modality of treatment in boys with balanitis xerotica obliterans, but now a days this newer technique of foreskin preputioplasty with steroid injection has gained popularity because of less invasive procedure and less complication(chance of meatal stenosis is less).
Although there are chances of recurrence of symptoms and the child may need repeat preputioplasty or may be circumcision, but the incidence is very low.So I would prefer the newer technique preputioplasty over circumcision.
Regards

Thank You very much for the answer. Just would like to add that he already got some problem with passing urine the urine first make a kind of ballon under the skin and then going slowly off , is there a chance that this tiny already whole for passing urine will be completely closed or you think in 2-3 weeks till the operation/preputioplasty he should be fine? Is the preputioplasty refund by NHS?

Brief Answer:
complete blockage of the passage in next two week is unlikely

Detailed Answer:
Thanks for the follow up

The ballooning of prepuce in phimosis during micturition is a common finding.Your surgeon has mentioned that there is BXO but this is a slowly progressive condition and I don't think that it will progress to such an extent within 2-3 weeks, causing complete blockage of the passage.
So I won't get overly worried for that.
Your next query was that whether this procedure is refundable by NHS or not.I am extremely sorry, but I don't have idea about this and you have to make a call to the NHS office regarding this or you can get an idea from your surgeon also.
Wish your son good health
