Is Increasing Dosage Of Klonopin Advisable For An Intermediate Metabolizer For CYP2C19?

Question: Hi,

I am on Lexapro 5 mg, Klonopin 3 mg, Keppra 1000mg and Amisulpride (Solian) 100mg. My psychiatrist decided to rise 6 days ago Klonopin from 2 mg to 3 mg. I am intermediate metabolizer for cyp2c19 and my question is: this condition makes the metabolism of Klonopin more slowly and then i have to wait a liitle bit more to reach steady plasma concentrations?

It's not the first time i ask this question but i ask it again because i received the following explanations: 1) Klonopin is a "short acting drug" (!!!), i've been told here, and excuse me, but it's not true. and 2) "for benzodiacepines steady state plasma concentrarions have not an important role in total concentration of the drug". It is not was i was asking, i just wanted to know if this steady stade can be delayed by my metabolism (Intermediate). I can say that the answer 1) is a little bit incredible.

With due respect, i would like to have certitude in the answer and due attention, since i am here because my psychiatrist don't like explanations y since i believe that doctors here are often good and i very often rate them well.

Thanks for your comprehension and i really hope my question is clear.

Brief Answer:
slow metabolisers need more time.

Detailed Answer:
Hello
Welcome here.

In my opinion, a slow metaboliser will definitely need much more time to achieve steady state plasma concentrations.

As you are intermediate metaboliser of CYP2C19, you will need more time to achieve steady state concentration of klonopin as CYP2C19 is involved in oxidation of benzos.

Klonopin is not a short acting drug. I would say its an intermediate acting drug. Long acting drugs are librium and valium, while short acting drugs are xanax, temazepam.

Hope this clears your confusion to some extent.

Thanks.
