Question: Hello
I hope to get an opinion from u yesterday I had a stressful situation with my daughter she had gotten hurt at school thank good she is fine now but after that I noticed that my heart was beating fast so I went to a triage where they took blood check traponin and ECG my heart rate at the time was 141 and blood pressure 136/90 they did an ECG but said because I am in a high panic mode it will not be accurate they did keep me on the monitor for awhile gave me 1 mg of Ativan and slowly it started to come done the dr said he believed it was not cardiac related but panaic attack th blood work came back normal
My blood pressure and pulse are usually I. The 80 when resting I am concerns that they r missing something or is this really panic and how can I get rid of it

Brief Answer:
Your symptoms are not cardiac related.

Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

I carefully passed through your question and would explain that your symptoms seem to be related to anxiety.

Your resting ECG and troponin have resulted normal and have excluded possible ischemic changes. From the other hand the fact that all your situation has improved by Ativan (which is an anxiolytic drug), is another argument if favor of a panic attack.

You should know that anxiety, physical or emotional stress activate the sympathetic nervous system ("fight or flight response"), which lead to a load of chatecholamines in your blood (adrenaline and noradrenaline), thus leading to increased heart rate and blood pressure. This is quite a normal physiological reaction that happens in every healthy person.

Sometimes, increased anxiety (like in panic attack), can cause a more prolonged symptomatology (high blood pressure, high heart rate including ectopic heart beats, shortness of breath, etc.), but this is not serious and harmful to your body. And it does not indicate any possible underlying medical disorder (including heart disorders).

So relax! You have nothing to worry about!

If these episodes occur again, I would recommend performing thyroid hormone levels to exclude possible thyroid dysfunction, which could also trigger this symptomatology.

Hope you will find this answer helpful!

Best wishes,

Dr. Iliri

Hello
Thank you for your response
The ECG from yesterday was so crazy they didn't even take it in to consideration because I was breathing so fast but they kept the monitor on me to watch my heart
The blood did show low potassium that they said I should have repeated
Today I feel some of the same symptoms alittle anxious thinking why my heart is beating so fast
Usually it beats 75-89 but at rest it is high can I bring having constant anxiety

Hello again!

Thank you for the additional information.

You should know that low potassium levels can induce cardiac arrhythmia, tachycardia and ectopic heart beats.

From the other hand ectopic heart beats and tachycardia can trigger a panic attack. Because palpitations and panic attacks are strongly connected, leading to exacerbation of each other.

So, I would recommend repeating your blood electrolytes, in order to be sure that anything abnormal is going on.

You may need intravenous potassium, if potassium levels result low again.

Meanwhile, I would recommend eating bananas and prune juice, which are known to contain a lot of potassium.

Hope you will find this answer helpful!

Detailed Answer:
Hello again!

I can assure you that your symptoms are not cardiac related.

I would recommend eating normally. As I explained bananas and prunes have a lot of potassium. If they fond low blood potassium levels, yesterday, you can eat these fruits today to avoid low potassium.

I would also recommend avoiding caffeine intake (coffee, energy drinks, etc.) and alcohol, because they are known to trigger anxiety and cause palpitations.

Hope to have been helpful!

Wishing a nice weekend,

My answer as follows:

Detailed Answer:
Hello again!

Here are my answers to your questions:

1. Yes, you can workout. Just avoid straining physical activity, because as I explained it can activate sympathetic nervous system, causing an increased heart rate and blood pressure.

2. Yes, you can go on vacation in 2 weeks. Nothing bad is going to happen. Just enjoy your life and go away from bad thoughts.

3. As I explained, I would recommend avoiding energy drinks containing caffeine or taurine. If there is not caffeine or taurin in gaterade you can take it. Anyway, I would not be a fan of these supplements.

Hope to have fulfilled all your expectancies!

Greetings!

Hello
So I have uploaded my blood work also I have been monitoring my blood pressure and heart rate every half hour and 3 X within 30-60 second of eachother
530 readings
127/82
130/84
110/80
6pm readings
147/87 102 Bpm
115/86 94 Bpm
142/85 92 Bpm
128/78 81 Bpm

Could it be that I am developing blood pressure problems ?
I know the sr said he thought the heart rate thing was because of anxiwty and low potassium but not cardiac issue ?
I am slated to go in for a echo cardio gram tomorrow morning as well as loop test but I don't know if I should be alarmed and go to the ER with these reading s

Hello
Please see the most recent uploads done tonight

Brief Answer:
Nothing serious is going on with you.

Detailed Answer:
Hello again!

I carefully reviewed your uploaded ECG and would explain that they are quite normal and do not indicate any cardiac arrhythmia or conductance disorders.

Regarding your BP values, they do not indicate high blood pressure. Most of these values (except for 142/85 and 147/87) are within the normal ranges.

But, as I explained a slightly elevated blood pressure could be related to anxiety.

I can reassure you that there is no need to go to the ER for this purpose, because this is not a serious medical condition.

I would recommend you to closely monitor your blood pressure for a more prolonged time (up to a week), usually in sitting position after 10 minutes of total relax and write down those values for 7 consecutive days. Then you should discuss with your doctor about these values.

If your BP is above 135/85mm Hg most of the time for an entire week, it can indicate possible beginning hypertension.

Anyway, I would recommend trying to focus on your anxiety and its management, in order to avoid this triggering factor of hypertension and tachycadia.

I would also recommend reducing salt intake in your diet (which can raise blood pressure) and avoid caffeine and energy drinks intake, which can trigger anxiety.

Hope you will feel better after my explanations.

If you will have any other uncertainties, please do not hesitate to ask me!

Regards,

Dr. Iliri

Hello
I have just come back from doing my echo and they said not to worry it looked ok
I am also wearing a holtet monitor right now that records stuff if I feel a symptom then I push record but it has already recorded something on its own that scares me
Can I still work out
Can I stil take the adivan if needed and should I do my regular routine
Is it something to be concerned about if it records on its own
Will I be able to fly if they say I need medication for fast heart rate

Brief Answer:
My answers to your questions:

Detailed Answer:
Hello again!

I would explain as follows:

1- Yes your symptoms do not seem to be cardiac related. Your performed ultrasound is another argument in favor of this fact.

2- The Holter monitoring, will record on its own. It may also record artifacts (which are caused by the movements of electrodes during body movements). They are easily recognized by your doctor, and have no medical significance.

3- Yes, you can take ativan during this test. It is a anxiolytic drug, but it has not any effects on the Holter monitoring (besides helping you calm down). It is necessary writing down the time when you will use it and show it to your doctor.

4- Even if you need to take beta blockers for the heart rate, you will be able to fly. They are not a contraindication to flying or going on holidays.

So relax! Everything is going to be Ok! I can reassure you that!

Hope you will find this answer helpful!
