Question : Hello,
I had a left shoulder total replacement on August 26. The next day, I started having sharp pins and needles feeling in my right foot directly between my big toe and 2nd toe. As days progressed swelling began in the right foot and soon carried over to the left foot with similar symptoms. Now I have pins & needles, then a burning sensation, and the redness is creeping up both legs. It’s not super sensitive to touch with pain, but the pins & needles can be really severe. No standing for more than 10 minutes. Many doctors seem and tests done; no cardiology issues, no vascular problems, no diabetes, no neuroma, no Rheumatoid arthritis, Lupus, etc. Any help please. It’s been 8-weeks now with same swelling in foot, ankles, up calves. The ankles are really tight. Help

Thank you for the response Dr. XXXXXXX This diagnosis is the same as a rheumatologist that I saw some weeks ago, however, the triple phase bone scan was done on me and there was nothing seen by the radiologist that is typically indicative of CRPS. Also, the pain is not nearly as intense as what I hear and read about other people with it that cannot even have any clothing touch the skin. My symptoms are not near that pain. Could it be that I am in an early stage? I will say that the only few things that bother me a lot are the following:
1. On the right foot, where it started the day after my left shoulder replacement surgery, it feels as though there is something of a thread from my sock between my big toe and the second toe, but there isn't anything. I don't feel anything in the bottoms of my feet, so I don't believe that it is Morton's neuroma. And the other thing that is painful is the ankle joints as they are very stiff, and feels like a tight wrap is surrounding each ankle. And finally, the discoloration and red dots on the shins are actually progressing upwards, and the hair on my legs is going away. Why is this?

So my questions are as follows: 1. Will this CRPS subside based upon what you hear from me and what you see on the photograph? 2. Is my CRPS a mild case and will it get much worse? 3. Or is this actually CRPS, but rather something else? Your kindness is wonderful and may God bless you forever!

Also Dr. XXXXXXX the doctor prescribed pregabalin 225 mg @ 3X per day, but I seem to be having some odd behaviors of hallucinating as well as some over eating with an appetite. Is this common and will these side effects subside the longer that I am on the medication? The redness on my legs does not itch at all, and the swelling on my feet and ankles is less swollen than yesterday, and even since two days before, I am hoping that it is going away from me. Can CRPS come and go day by day?

I am sorry but I havent been able to see the photograph.
yes u are on a fairly high dose of pregabalin, possibly it can be reduced and built up again once you tolerate it better.
also wanted to know whether an MRI or any other imaging was performed for the ankles to rule out other causes like arthritis/swelling inside joints-synovitis/periarthritis.
so if it is crps,yes the symptoms may vary on and off and it should definitely get better as you are already on medication for the same.
also I would like to know whether you have had a chest x ray done, as the only other thing I am concerned about is lofgrens syndrome which can present with ankle pain and swelling with red nodules and lymphadenopathy on the chest.
I hope markers to check inflammation like ESR and CRP have also been done.

ok so then I think we need to wait and see the results of the medication you are on.
are u also taking any NSAID for the pain/swelling, if yes does it give any relief. just checking that to see if the problem is related to the neuritis-CRPS or inflammatory.

Dr. Aggarwal,
There was many, many blood markers run on me over the past 8 weeks with no issues on CRP, ESR, etc. There have been chest X-rays and they were normal with the exception of some pleural effusion in the lower left lung. There are no nodules on my chest. MRI done on ankles shows no major issues. My ACE is a value of 53. I have some trace protein in my urine with slightly high levels on BUN/Creatine. I just am frustrated as my diagnosis is still up in the air. My ankles are so very stiff and tight.
