- Your triglyceride level is too high (six times above the maximal normal range) and this needs an emergent treatment, because it can lead to acute pancreatitis, which is a lifethreatening medical disorder.
- Your HDL levels are low, and Total cholesterol/ HDL ratio is too high (8.8), which indicates that you are at an increased risk for cardio-vascular disease.

For this reason, I would recommend consulting with a dietitian in order to help improve your situation by diet (low fat and sugar intake) and also start taking fenofibrate or cyprofibrate to help lower triglycerides and cholesterol levels.

Besides your lipidogram, the most concerning thing is increased D-Dimmer plasma levels , which indicates blood clot formation. I would like to know more about your situation:
- Do you have shortness of breath, chest pain or cough?
- Do you have leg swelling, which could indicate any possible deep vein thrombosis?

To answer your questions, I’ve not had any leg swelling or pain. I am a smoker so I cough phlegm up regularly. I don’t have chest pain but a general discomfort that radiates to my back. I don’t use any pain killers.. not even Tylenol. Even with a low sugar diet along with metformin and glyburide, seems my sugars are still out of control. I have shortness of breath when I’m active with short walks or stairs. Something to note is I have chronic and sometimes very heavy vaginal bleeding. I was told it was common with PCOS and I refused birth control because I don’t want to risk blood clots. Also, my face is consistently red. I’ve had renal cell carcinoma. I only have one kidney now. I am constantly fatigued. I rarely sleep more than a few hours at a time. Average about 4-5 hours a night. I do have a Chek2 mutation if that helps. I’m trying to give you a big picture.. I’m also on humira for spondyloarthritis, duloxitine and aripiprazole for depression, and levothyroxin for thyroid replacement. Hope this helps some.

Coming to this point, considering the shortness of breath, I would recommend performing a lung angio-CT scan in order to examine your lungs better.

Regarding your blood lipid profile, it is important making some changes in your diet and taking cyclofibrate or fenofibrate (as above mentioned).

I would also recommend performing an abdominal ultrasound in order to examine your liver and pancreas (in order to investigate for possible fatty liver.

Regarding your blood sugar, I would recommend discussing with your endocrinologist on the possibility of switching to insulin, considering the fact that your liver enzymes are elevated and your blood sugar in not under control (and this is seriously affecting your cardio-vascular system, increasing the risk for stroke or myocardial infarction).
