Hello and Welcome to 'Ask A Doctor' service.
I have reviewed your query and here is my advice.

As per your complaint it seems that you are suffering from fatty liver disease.Basically there are four stages of fatty liver but as per the details given by you it seems to be Grade 1 fatty liver-This is where excess fat builds up in the liver cells but is considered harmless(Change in echo texture is because of deposition of fat only). There are usually no symptoms and patient may not even realise until you receive an abnormal usg result.In this stage the damage caused to liver is fully reversible with proper life style modification.

Natural antioxidant and potent liver rejenuvator like sylimarin can be taken once daily every day for 3 months to hasten up the recovery.Its over the counter.

Extreme fatigue, bloating etc may have a reason/cause.
Blood work, Ultrasound of abdomen are normal. It would have been helpful if you could post the blood tests done so far.
Do you have any other symptoms like coughing, change in bowel habits, urinary symptoms, anemia, edema over legs, change of voice,Constipation,Appetite loss or swallowing difficulty. Detailed history makes it possible to have judgment about the proper clinical evaluation and further specific investigations to know the cause.

Blood tests for CBC, diabetes, kidney, pancreas and thyroid functions.Tests of stool and urine. Any additional tests as per the symptoms and signs. Also watch for psychological changes if any.

As far as test is concerned a CT scan of upper abdomen is good idea.A much better picture of liver is obtained in CT and proper contouring as well as condition lf hepatocytes could be seen in it.
In follow up kindly mention-
Exact site of pain whether upper lower or mid belly?Whether its on right side or left side?
How are your bowel habits?Do you suffer from constipation or change in bowel habits?
Is pain constant or episodic?
History of gas formation or acid reflux?
