Question: I have had stomach pain for years but doctors can never find the cause. The problem is often exacerbated by things like alcohol or spicy food. After not drinking for over a year, I foolishly thought I may no longer be at risk of stomach pain from drinking if done in moderation. I drank several glasses of Saké, only 15% alcohol, on the evening of 11/7. I woke up feeling fine at first, with no hangover, I ate, and then a few hours later I became horribly ill. My stomach pain was pretty unbearable, and I vomited several times throughout the day. I vomited stomach acid and white chalky substance. Days later even today my stomach still hurts very much in my left abdomen. My left Abdomen appears to be physically swollen slightly. My urine appears to have oil in it and be a kind of chalky. In retrospect, I obviously realize it was foolish of me to drink because I've had past experiences like this from even light amounts of alcohol, but what I'm interested in knowing is anyone could have a suggestion as to what may be the underlying problem here? I mean I have stomach pain on a regular basis, pretty much regardless of what I'm eating or drinking. I'm 35, and I have a history of diverticulitis on both my mother and father's side. Both my maternal uncle and my father have had bowel resection surgery. Anytime I've brought that up to any doctors though it's been dismissed. I've had a colonoscopy, endoscopy (upper gi), ultrasound, and even a MRI I think, and no one can ever find the cause. I often feel very sick, but I never feel 'good'. I can remember what it feels like to be healthy, and I haven't felt it in a long time. It's been at least 15 years. So that's the first part of my question, and my second part is do I need to worry that I have some sort of permanent organ damage going on at the moment as a result of this current pain and strange urination?

Hello and welcome to Ask A Doctor services.
I have read your query and here is my advise.

* As per my clinical experience,
for the first part of your question, possible causes include :
recurrent gastritis ( usually food or water induced ), disturbances in stomach pH or else.
for the second part of question, possibility of permanent organ damage depends upon the primary cause of
current concern.

* Guidelines at present to deal with acute condition, life style modifications as well as medications are
mandatory such as
- drinking more liquids at sessions
- prefer easily digestible food stuffs
- avoid all spices, non veg, dairy fat, junk food
- avoid all aerated drinks, beverages including any concentration of alcohol drinks
- prescription of anti emetic as ondancetron and proton pump inhibitor as omeprazole ( to be consumed with
sips of water on empty stomach )
- avoid all sorts of analgesic including acetaminophen
- urine gets clear color tone within next 48 hours or so as there is metabolic upset as underlying cause
- avoid stress and anxiety

* So far as long term basis control is concerned, over and above dietary precautions, you need to follow
- regular walking , exercise
- meditations
- control of body weight
- avoidance of smoking or tobacco consumption if involved
- regular sound sleep of 7 hours
- stress control at home, work place
- regular doc visit for vitals evaluation, necessary lab tests ( like liver function and others ) as per the need
and doc advise
- for camera pill test, I would suggest that you discuss the same with your treating doc once this acute pain
condition is resolved
