Brief Answer:
Please upload a picture, keep the finger dry, clean with betadine.

Detailed Answer:
Hello Mr. XXXXXXX

Welcome to Ask a Doctor service.

Fungal infection can be diagnosed accurately by doing a skin scraping examination under micrscopy by KOH mounting. Please do visit your local skin specialist for accurate diagnosis of the condition.

If it is indeed confirmed to be fungal infection of the nail, then the treatment should continue for at least 6 weeks.

The mainstay of treatment is keeping the affected fingernail free from moisture at all times.

Cleaning the affected area with betadine solution twice a day.

Applying a local antifungal cream like clotrimazole or ketoconazole twice a day for at least 6 weeks.

If there is added bacterial infection (which is very common), antibacterial cream should also be applied.

Please upload a clear photograph of the affected finger here so that I can confirm if it is fungal infection.

And also please do visit your local doctor for prescription of the antifungal cream and if needed antibacterial and antifungal oral medicines.

I hope that answers your query.

Best regards

Brief Answer:
KOH Wet mount microscopy or fungal culture

Detailed Answer:
Hello Mr. XXXXXXX

Thank you for writing back with further details.

The best test to confirm the diagnosis will be fungal culture from the skin scrapings from the affected finger nail.

You must ask your skin doctor to request a fungal culture or KOH wet mount microscopy.

These tests will confirm if it is fungal infection and will tell us which fungus.

Antifungal treatment should be continued for a minimum of 6 weeks without fail, otherwise it will not get cured.

Fluconazole should be taken 200 mg on day 1 and 100 mg from day 2 to day 5, then once every week for 6 to 8 weeks.

Topical cream should be applied at least 2 times a day for 6 to 8 weeks daily.

It recurs again and again because you do not complete the treatment properly.

I have told the same in my earlier reply.

Please visit your skin doctor and proceed further.

Wishing you speedy recovery.

Best wishes
