Question: I am a 53 year old male. My symptoms began about three months ago and include the following:

Numbness and burning at base of neck
Shooting pains and stingers in upper back and near shoulder blades
Soreness at base of skull down the left side of neck down into left collarbone that comes and goes.
Muscle twitching that comes and goes particularly in hands, fingers eyelids and ankles almost all at rest
Body temperature regulation issues causing damp collar of my t-shirt after waking from sleep and now some anihydrosis on exertion because I did notice I do not seem to be sweating as much on exertion
Foggy head feeling with some slight dizziness on and off for the last three weeks
Dry throat upon waking up and sometimes during the day that wont go away
Frequent urination some days that sends me to the bathroom every 1.5 hours

Tests my internist has conducted in the last month to investigate my symptoms:

MRI of my Cervical Spine that showed some bulging discs but nothing significant
CAT scan of my neck to look at lymph nodes and all was normal
Chest X-Ray which was normal
Blood work which showed all normal except Vitamin D which was low. I am now taking a supplement

I am obviously very scared that I am developing a MND such as Parkinsons or MSA.

• More details about my story:
I was diagnosed with Benign Fasiculation Syndrome 23 years ago and I have experienced occasional muscle twitching ever since. In October of 2018 my twitching began to happen more often in my hands and fingers, eyelids, triceps and ankles. In January 2019 I developed a burning numbness at the base of my neck and across my shoulders. I also experienced shooting pains and stingers and then some soreness in my shoulders on and off. This is still occurring to this day but gotten better but I am still getting shooting pains, numbness at base of neck on my back and occasional soreness in my shoulder blade area on one side or the other. I wake up some days with a dry throat and sometimes I experience during the day. For about a month I was waking up with a damp t-Shirt collar. On mornings when I go to the airport and I walk a lot I feel like I am overheating and I don't seem to be sweating. Over the last three weeks I have experienced some mild foggy head feeling and very mild dizziness especially in the morning but on and off most days. I do have occasional soreness on my left side of my chin underneath on the bone and sometimes it feels like there is pain on the left side of my neck that starts at the base of my skull and goes down to my collar bone. I have been experiencing mild Paresthesia on the top of my head. Finally for three days this week it felt like I was urinating more than usual. I was literally going every 1.5 hours. It is somewhat back to normal now.

• Previous history:
Other than Muscle twitching none of this was happening before January 2019. My internist has run a battery of tests to try to figure it out and I did see a Neurologist who examined me and told me I showed no evidence of MND. He sent me for the C-Spine MRI.

• Current medications:
I take Crestor 0.5mg for Cholesterol and a Vitamin D Supplement.

The numbness and burning sensation at the base of the neck that you have is likely to be related to mild disc bulge in the cervical spine. Low levels of vitamin B12 can also contribute to it. Regular neck isometric exercises can help in decreasing any nerve root irritation.
The muscle twitching and foggy sensation can be due to low levels of Vitamin D3. Decreased fluid intake and electrolyte disturbances can also affect these. Proper supplements and electrolyte rich fluids will help in recovery.

Thanks for writing again.
I have seen all the attached reports. The changes in cervical spine MRI are minimal. A muscle spasm in the area can cause the symptoms. Taking oral muscle relaxant and continuing with Vitamin D3 supplements will help.
