Question: I am 64 years old and post menopausal. I’ve been passing large blood clots. I may pass up to 6-7 clots in a 12 hour period. Then it doesn’t happen again for months. What causes it?

Post menopausal bleeding can be due to benign (not dangerous causes) in very many persons but it always needs to be assessed.

It can be due to (1)atrophy of the vagina or uterus- atrophy means that waste away or breakdown- this is due to the decreased hormones in menopause
Persons with atrophy of the uterus have usually been menopausal for about 10 years.
Atrophy is treated with hormone therapy

(2)uterine or cervical polyps- polyps are outgrowths/protrusions that can occur, they are benign (not cancerous growths ).
Polyps in the cervix, if seen in the gyna office can sometimes be removed there and sent to the lab for assessment.


(3)endometrial hyperplasia- the endometrium is the internal lining of the uterus. Endometrial hyperplasia refers to thickening of this lining

(4)cancer of the uterus, cervix, vagina

Normally, a biopsy of the endometrium (inner lining of the uterus) is done in persons with post menopausal women with bleeding. This is to check for dangerous changes to the uterus, this can be done in gyna office.
Your doctor may also suggest an ultrasound

Even though the bleeding comes and goes and is due to non dangerous causes for most persons, it would be best to be seen as soon as possible.
