Question: I am 24 years old (male) height 5feet 8inch , weight 69kg. I look very small, lean compared to people of my age and and even small compared to 8 years younger brother of mine, my face, my chest looks like half grown adult, no proper beard growth -only at chin, no chest hair. I grew up like being odd lean one all my childhood facing this problem. When i turned 21, I got bum fat and belly fat. I believe my genital organ is small. I don't have the thick skin on my chest, my fingers and toes look small and week.I couldn't lift much weights in gym. I am diagnosed with gynocomastia(acute) recently. I don't have thyroid problem. Are these symptoms related to growth harmone deficiency or pitiutory tumour?

Other problems i had are:
daydreaming, feel tired very soon if i do some physical work, no concentration(hardly 2-3mins), lack of desire, staying away from social life, no self esteem

Brief Answer:
possible growth hormone alteration. pitutary tumor less likely.

Detailed Answer:
Hello and welcome to Healthcaremagic.com
I have read your query and attached report of lab data, here is my advise.

* As per my surgical experience, the issue with current symptoms narrated here seems
more likely to be related to growth hormone level alteration or other reason as the
testosterone levels and other parameters are quite okay.

Kindly provide me further details so that I can answer your query completely.

1. Degree of variation in symptoms since onset in details.
2. Lack of desire is associated with erectile dysfunction or ejaculation problems ?
3. Any changes in voice ?

Awaiting for your response.
Regards.

Thank you doctor for responding,

1) I was thin, lean and always underweight all my childhood. At age 20, when I observed getting chest fat and stomach fat ,I went for gym few months increased my weight a little. After 6 months i started getting fat on my butt, belly and chest little more than last time. My weight increased gradually but there is not much of bone or muscle growth. I started working out rigorously for 3-4 months but the fat didn't go away. Didn't realize it was gynecomastia but later diagnosed.

I always had the difficulty in concentrating since my childhood, but started becoming worse from this point, couldn't remember much of what i studied and had hard time. From that point i started having the day dreaming all the time, while driving , eating , any physical work my body just work on it but i am never thinking what i am doing. The thickness of my hair is reduced. No chest hair is developed- soft skin all over the upper body, no proper mustache or beard (looks like half grown). I feel shorter than before and weak, gets tired easily if i had few hours of work, my chest, ribs, legs, especially my face, jaws looks smaller than compared to avg person of my age. Please let me know if you want to know anything in specific.

2) I believe my genital organ is small. I get this urge to masturbation every week sometimes too often in a week, if i don't do it, i will be having wet dreams over night. I have premature ejaculation, after 30 sec - to 1 min, my genital organ go small though i am not done with. I don't know the medical terminology much, i hope this answered the question.

3) Nothing of a big change as i observed.

Detailed Answer:
Hi
Thanks for the follow up.

* As per further details, there can be low level of growth hormone along with micro
nutrients deficiency such as zinc, minerals or else.

My suggestions at present

- Keep hydration level at best with plenty of liquids.
- Stay away from stress or panic of major disease suffering phenomenon.
- Consume balanced nutritious diet as containing carbohydrates, fat, vitamins, minerals
anti oxidants, proteins, green leafy vegetables.
- Do regular physical exercises.
- Genital size also varies in different individuals, so it does not seem to be of issue till
you are getting night fall or erection.
- Discuss the need of growth hormone evaluation and ultrasound of the testicles when

Can this growth hormone evaluation be treated ? If so does it involve any surgeries ? How long will it take for the treatment? Please advise me on this.

Thank you doctor

Detailed Answer:
Hi

* Treatment of growth hormone alterations are surely treatable in best way.

* There are panel of evaluations with growth hormone, gonadotropin releasing hormone
and other to consider by the endocrinologist.

* In majority of the cases, the medicines give best results.
Only in rare instances surgery may come in action.
The duration of treatment depends upon its severity and progress as per the clinical
evaluation.

Thanks.

Detailed Answer:
Hi,

* There are many more reasons apart from hormones such as deficiency of micronutrients, trace elements, cerebral neurons pathways handling fine balancing acts of the body including central
nervous system and its related organs.

* Once the Endocrinologist assesses you clinically, s/he may decide for further work up
with MRI brain or other parameters necessary as per the clinical requirement.

Thanks.
