How Can Hypotension In A Diabetic Patient During Atrial Fibrillation Or While On Erectile Dysfunction Pills Be Managed?

default
Posted on Fri, 8 Sep 2023
Question: Occasionally my BP drops to 75/50 and 80/40. Often after a hot bath; during atrial fib or taking erectile dysfunction pill (highest dosage suggested)--in the evening, without having taking the following meds: I take 5 mg Norvasc, Eliquis and flecainide 50 mg twice daily; and daily digoxin and Losartan. I have a pacemaker. I am diabetic on insulin but fairly well-controlled. I am a male 78. No congestive heart issues. Any recommendations?

Brief Answer:
About hypotension causes;

Detailed Answer:

Hello,

Atrial fibrilation, erectile disfunction pills, may cause hypotension but in a diabetic patient, your physician should check also for orthostatic hypotension and other symptoms of autonomic neuropathy (a chronic complication of Diabetes).

It may cause hypotension and special treatment are recommended in these cases.

Adrenal function is also recommended to be checked if there are other symptoms suggesting for it.

The erectile dysfunction pills should be avoided if no other cause of hypotension is found and if the hypotension is symptomatic.

So a detailed clinical examination and the remove of erectile dysfunction pills are suggested, except for the recommendations of your physician.

Hope I have answered your query. Let me know if I can assist you further.

Dear Dr. Mirjeta, thank you for your observations. Is my blood pressure of 75/50 and 80/40 in the situations described in my inquiry dangerously low and could ( possibly or unlikely) precipitate stroke or heart attack or other harm?
Keep up the very helpful work!

Detailed Answer:

Hi,

Yes, the blood pressure under 100/70 mm/hg at your age is considered dangerous and can precipitate stroke or heart attack.
Hypotension events should be avoided at your age.
