Brief Answer:
Need to check the rectum and anal region

Detailed Answer:
Hi
thanks for your query.

These symptoms can be explained by some local lesion at the anal/rectal region. Most commonly an infection. Other possibilities are hemorrhoids, fissure, polyp or even tumor.

The first and foremost thing to do is office examination by a doctor, preferably gastroenterologist or surgeon. A routine stool exam may also be required. If it looks like an infection, a course of antibiotics should suffice. If anything suspicious, then a colonoscopy is in order.

Hopefully these will clinch the diagnosis and then it can be treated accordingly.

Hope this helps.
Regards
