Question: I had a CMIA HIV 1&2 Ab/p24 Ag test done at 38 days post unprotected sex. Test came back negative. How accurate is this test? Is it conclusive or do you suggest getting tested again?

Brief Answer:
Specific test.No need for repeat test.

Detailed Answer:
Hi
Thank you for query. I am Dr Bharatesh D Basti answering your query.
The Combo test which you have undergone has very specificity of more than 99%.
i.e it suggests true negative : it truly gives negative results if they do not have disease .
The repeat tests are done mainly many times to relieve the anxiety.
So you can allay fears of HIV for that episode.
I do not feel there is necessity for you to undergo test again
You can get back to me for any clarifications.
Thank you
Dr Bharatesh

Thanks for your answer. My concern is that now, 10 weeks post unprotected sex, I have significant pain in my neck that continues through my shoulders. This has only been this way for 4-5 days. I also went to the dr. As I developed a rash in crease of groin and thigh. Dr said it was yeast infection due to hot weather and sweating, I am also a little over weight. But the rash cleared up quickly within 5 days. Would these two things change your opinion? Also, is the test still 99% accurate even only after 38 days?

Brief Answer:
Non specific and not related to HIV.

Detailed Answer:
Hi
Thank you for follow up query.
The pain in your neck may be due to gastritis or due to musculoskeletal problems. And it is not related to HIV.so do not bother.gastritis may be due to junk foods spicy and carbonated drinks or due to anxiety. Please avoid all these.Have short frequent meals.try with antacid chewable or syrups.also take H2 blockers like ranitidine.if does not subside consult doctor for ruling out other causes.
As your doctor has diagnosed the lesions in the groin is common due to fungal infections.which is common in humid conditions ,overweight and diabetics.try to rule out these and treat it.Please take following precautions.Use loose cotton clothes and inner wears.Keep area dry.Apply antifungals like terbinafine which is fungicidal.Once subsides use regularly antifungal dusting powders like candid or absorb powder to prevent recurrence.Maintaining moisture free area is most important to prevent recurrence.
This is also not specific to HIV and not related to it.
The Combo test is positive for antigen by 3 weeks after exposure. As both antigen and antibody are not present it is 99% sure of negativity and you can allay fears of HIV.
You can get back to me for any clarifications else please close the discussion and rate.
Thank you
Dr Bharatesh
