Question: Hi there, I was diagnosed with crohns in 2016 and I have been on Humira ever since. That is until this February 2020 when my script expired and then COVID started. I have an appt to see my GI on 10/13/20. However my pain continues to get worse. I went to my PCP 2 days ago and all she could do was prescribe me 5 days worth of prednisone. The pain I am getting is just above my waistline an both sides, like my lower abdomen. I keep contributing it to inflammation. I had a fever off and on in the beginning but now I don’t. I am experiencing night sweats occasionally. Lying down flat is my favorite position right now. Anything with bringing my knees towards my chest increases pain. When I drink any liquid I get a cramping like pain just below my sternum for about 10-15 sec. I am currently taking omeprazole, benefiber and just started the prednisone yesterday. I have had this increased pain for about 4days now. I guess my question is.... does this seem related to inflammation from my crohns?

Detailed Answer:

Hi
Welcome to ask a doctor
Thank you for the query.
I understand your concern.
Well yes it could be related to crohns as the kind of pain you say and that you are off your humira for quite some time so a strong possibility of relapse of Crohns is there.
Worthwhile continuing steroids and moving to liquid diet.
Worthwhile doing a cbc esr crp fecal calprotecting and abdominal imaging like a sonography or a ct scan.
Restarting your humira is important when other possibilities are ruled out.
Can slowly institute soft diet and see how you feel.
also can try mild antispasmodics like drotaverine or mebeverine if available locally twice a day.
hope this answers your query
wish you a speedy recovery
best wishes

Thank you for your help on this. I just did some checking on the 2 anti spasmodic medications you recommended however they both require a prescription. Do you have any recommendations that I can get over the counter?

Detailed Answer:

thank you for the query.
SORRY, well i think my prescription wont work there as my registration is invalid there.
i think try taking paracetamol (OTC)650 thrice a day for 3 days and then reduce dose as you feel better worth visiting your general practitioner and ask for an anti spasmodic again.
best wishes
