Brief Answer:
Investigations needed

Detailed Answer:
Thanks for using the Ask a Doctor service.

I have gone carefully through your query and can see the details of your medication and diet plan. However, that is not sufficient to comment why the given measures are not enough. I would need you to check the following:
* Blood for fasting and post prandial blood sugar (preferably checked on the same day), HbA1c, Creatinine

Based on the above I would be able to guide you further.

Brief Answer:
Blood sugar has been high for long

Detailed Answer:
Thanks for writing back. I have checked the attached reports. A HbA1c level of 10.5 is huge indicating an undetected high blood glucose for a prolonged duration even though the episodic blood sugar measurements fail to pick it up. In such a case medication would be tricky as hypoglycemia needs to be avoided. In such a case it would be best to visit an endocrinologist/diabetologist without delay.

Thank you sir for the advice. Yesterday i met doctor and he suggested Gemer 2mg and supranex vitam tablet. I am continuing with his suggestions. Is the above tablets will be controlling my blood sugar or can i continue with Triglonyse 1mg and Janumet 50/500? What is the difference on these both. Please advice.

Brief Answer:
Please follow the directions of your treating physician

Detailed Answer:
Thanks for writing back. Sorry for the inadvertent delay in responding. I would insist you to try the current medicines suggested by your doctor. As you can understand, the control with the previous medicines was inadequate and therefore a modification in the treatment regimen is needed.

If I were the treating physician I would perhaps have chosen Janumet 50/500 with Gemer 2. But please discuss with your treating physician before deciding on what to take. I would rely on his judgement. Close follow up would be needed.
