Hi Dr
last week I was with a woman of Unknown HIV status. She gave me a blow job and then I moves my penis on her butts. At the time I was with her I was sure that I did not penetrate her anus but then when I stayed alone by myself I kept on overthinking and asked myself what if my penis slide and penetrate her anal opening without me feeling or noticing it. My questions are: ASSUME that the woman is HIV positive (Assume)
1) What are chances of me getting HIV if she suck my penis? (NB: I have herpes and I am on 500 mg valtrex daily as prophylaxis to prevent outbreaks and I have not got any outbreaks for almost a year).
2) Do you think it makes sense that I penetrated her anal opening without feeling or noticing? I believe anal opening is tight and needs work to relax it and there is no way my penis penetrate her without me and her feeling it. I asked her and she told me that she did not feel any penetration.
3) Say for example that my penis touch her anus from outside, can I contract HIV if she is positive. Thank you.

I want to add that Assume that my penis touch the rectal opening from outside and there is rectal secreation on the outer part of the rectal opening, I would definitely feel any wetness but I did not feel anything wet. Also as I mentioned in the previous question that anal opening is tight and it is not fully patent and and I need to open her butts cheeks to locate the anus, furthermore my penis head will encounter some resistence and I have not felt any of that. Actually the whole time including her sucking my penis and moving it on her butts took less than a minute.

All the sex workers are not necessarily HIV reactive.
As per CDC, XXXXXXX USA Oral sex carries a low risk of HIV transmission when compared to anal and vaginal sex.

So your blow job with her puts you for low risk for HIV.
Anal rubbing with penis carries a negligible risk for HIV transmission.
As you rightly pointed out anal sphincter offers a high degree of resistance for penetration. Without perception, one cannot enter into other's anus, even if it is patulous. So your HIV risk is not high. Presence of STI infections in the anal rim can cause infection during rubbing.
Rectum, as such, has no secretions.

Actually I am not in XXXXXXX and she is not a sex worker. I like reading and I read in a study that HIV is inactivated in saliva even if this saliva contains blood due to certain enzymes in the saliva and there was no reported cases of HIV by only blow jobs but I am not a doctor so I am not sure if this information is correct.

When you mention abiut other STI infections in the anal rim, are you referring to hepatitis B and C , gonorrhea, Chlamydia, herpes and syphilis? I am not sure of hepatitis B AND C can be found on anal rim though.

Is anal mucous that is lining the mucosa is considered a secreation and contains HIV virus?

What about deep kissing or french kissing? I read there is only one case reported by CDC in 1999. And also according to CDC that this reported cases is still suspected to get HIV by other than french kissing such as condom tearing or slipping.

I believe I will notice if a blood from the partner comes inside the mouth, I will at least feel the taste of that blood. Even though I read recently in a study that even when there is blood inside the mouth if infected partner this will not transmit HIV, however, I am not sure if how strong is that study. So from your medical experience:
1) Have you ever meet a case got HIV by deep kissing even when there is blood in the mouth?

According to what you told me that my risk is negligable, Can I rest assured and make sex with my wife or do you think I need to get tested? Thank you.

To be honest. I stopped making sex with my wife after the exposure that I discussed in the first question in addition to deep kissing that I mentioned in the second follow up question. However, I started making sex with my wife again when you told me that my risk is negligable.

The presence of minor illness of the oral cavity would not have been recognized by the individual in most situations like Gingivitis, Scurvy, etc. The contamination of blood is not necessarily visible obviously. There may be the microscopic presence of blood cells in the saliva that could be infectious with a deep kiss or french kisses.

If the HIV Positive individual is already on treatment with an effective regimen of ART, and in a fully (Virus) suppressed condition of HIV infection, then the individual remains non-infectious even if there is direct contact with the blood of that individual.

So it is possible with a deep kiss and the contact with saliva contaminated blood to remain HIV free in spite of deep kisses. Taste of blood is not a criterion for the saliva contaminated with blood, especially if it is in microscopic quantity.

I have not come across anybody to say an individual got the HIV infection through only deep kissing as they were having multi-various sexual activities at a single time.

Regarding your risk, I never told your risk is "nil" or you are free from any risk. I can quote the guidelines from bodies like CDC or WHO, but at the same time, medicine is not Mathematics to have the same and constant answer. Your certain activities were having low risk and other activities are having negligible risk and any high-risk activities. That too based on your words only.

Only after ruling out HIV infection with appropriate tests after the window periods for the tests, it'll be better to resume unprotected sexual activities with your wife.

I have an information and I need to confirm it with you. In genital herpes in the absence of manifestations or recurrent episodes there is a possibility of HSV transmission due to the viral shedding, especially before and after the attacks for few days. But in the absence of lesions there will not be any inflammation usually in HSV infection. So it is unlikely, in the absence Herpes manifestations, the chances of HIV infection to be increased.
So not to worry about the past HSV infection, especially as you are on suppressive therapy and no recurrence for more than six months.
