Hi Doctor, One Of My Closed Relation Is Severly Suffering

Question: Hi Doctor,

One of my closed relation is severly suffering with breathing issues.She is on oxygen currently.All of a sudden one day her condition worsen and she was being admitted to hospital.Now hosiptal is discharding her but she will remain on oxygen,we need a clarity what is the root cause for her health issue and what XXXXXXX we can do to improve her condition.Kinldy guide us.I am attaching her report here.

Thank you for posting your query related to Mrs XXXXXXX and uploading the relevant reports, I have gone through all the attached reports.

There are some problems that are very clearly evident from the reports that are quite serious.

1. She has had a heart attach recently - Her high sensitivity troponin levels are indicating this, she has long standing high blood pressure/aortic valve problem that has caused her heart to become bulky, which requires more than usual oxygen levels and when that is coupled with her lung issue, heart attack can happen easily.

2. Her lungs are vadly damaged due to some infection or something that might happened long ago, there is interstitial fibrosis in the lungs as decribed in the X-ray, this reduces the oxygen exchange in the lungs, so the effective oxygen mixing with the blood with normal air will be low, that is why the doctors have suggested to keep her on supplemental oxygen.

These two issues don't play well together and can make it quite difficult to handle.

The root cause cannot be assetained at this stage, it might have been due to some viral infection, or it might be a genetic predisposition to pulmonary fibrosis and her heart enlargement is most likely due to high blood pressure or back pressure caused by the hardened aortic valve.

Unfortunate thing is, there is nothing much that can be done to reverse this condition, oxygen will help her lead relatively comfortably, but improving maybe difficult considering the new heart attack.

I recommend you to discuss these points with her cardiologist and pulmonologist and see what is the best way ahead.

Thanks doctor for your reply. As a follow up question i wanted to ask now a days she is murmuring also while sleeping,what could be the reason behind that.

Murmuring in sleep is harmless in otherwise normal persons and as such does not cause anything major in sick people as well as mumuring in sleep is not by itself some problem, but it is an indication of illness.

In the present situation as your near relative suffered from a major health issue, that could have precipitated this murmuring to occur now.

Stress, fever, reduced oxygen to brain can all cause this murmuring/mumbling is sick people.
