Question: Hi Doctor. I have been using Tab Thyronorm 100mcg since 1 year for my hypo hypothyroidism problem. Recently I have been tested for TSH and the level was increased. Could you please assist.

I've checked the reports. In hypothyroidism, thyroid hormone replacement is started at an anticipated full replacement dose and dose adjustment is done according to TSH levels.

The change in TSH levels always lags behind the change in dosage by about 4 weeks so the dosage should be adjusted in small increments after no less than 4-6 weeks.

I would advise you to increase levothyroxine dosage by 25 µg/day (so now you can switch to 125 micrograms per day) and get TSH done after 12 weeks.
It can be escalated likewise if TSH remains high.

In the long run, if the thyroid levels persist to be high, kindly visit your endocrinologist.
