Question: Hey doctor,

For the last year I’ve had this burning pain when I urinate. It tends to be a lot worse if I don’t drink enough water or fluids in general and I also feel pain sometimes when I masterbate too frequently right in the centre of my pelvic region afterwards. I went to my doctor about it when it first started happening a year ago and I had a ultrasound last year of my urinary tract along with a bunch of urine and blood test. I recently re-did the urine and blood tests last month which were normal and I was referred to a uroglosist who I saw and he just thought it was normal and that I shouldn’t worry about it. What exactly can this be I just find it super irriatating. I’ve also never had sexual relations so it’s not related to an infection or std what else could it be? Could I have had bladder/prostate cancer this whole time is that possible for someone my age? Is there anything else I should get tested should the ultrasound be re-done?

from the symptoms that you have described you may have a condition called prostatitis. this is the condition wherein there is swelling or infection of the prostate.

this is diagnosed by doing a digital rectal examination in the clinic a semen culture to determine what bacteria is is present and what antibiotic is needed to treat the infection.

prostatic massage could also help you every two weeks to relieve the pain in the prostate.

I read up about chronic prostatits and how it’s hard to cure once it’s chronic? Why would my urologist not want to do any further testing he thinks because the urine tests came out normal that it’s nothing to worry about?

thank you for your follow up question. masturbation would not worsen the condition but it may lessen the swelling because of frequent ejaculation. but it should not be done more than twice a week.

it is really hard to say why your urologist did not do further testing but you may go back to him and ask him about prostatitis and he may do the necessary tests for you.

there is no correlation between prostatitis and prostate cancer development. it is possible that the bacteria is still there causing the symptoms because there was no treatment given or the bacteria that was present before could illicit changes causing the build up of inflammatory cells in the organ causing chronic prostatitis. yes some patients with with chronic prostatitis are not responding to treatment and are always in pain. but these subsets of patients are uncommon and most patient suffer from acute prostatitis and are readily treated with antibiotics.

Hey doctor,

Thanks for the follow up with those tests would a normal gp be able to do them, going back to the urologist directly for tests is quite costly in my case? Also how exactly does one get Prostatitis I haven’t had sexual relations before but I have used a flesh light in the past could that have maybe had bacteria or fungus if I hadn’t cleaned it properly and caused the infection I’m not sure if that’s a cause or I masterbated with washing my hands? Also in terms of tests if it were an infection and especially with the burning urine wouldn’t the multiple urine tests I had done shown some sort of bacteria, I had them sent to them lab as well and cultured? In terms of treatment if it were that is antibiotics the only treatment option is there any alternatives natural or otherwise? The issue is I have really bad gut problems a lot worse than this. My symptoms are pretty mild and more so an annoyance at this stage but my gut issues are very bad and antibiotics really make it a lot worse my gastroenterologist has me avoiding them completely unless im super sick but I haven’t been on them in 4 years when my gut issues started after taking a month worth of doxycline for acne about 4 years ago.
