Hey Doctor, For The Last Few Months I’ve Been Having

Question: Hey doctor,

For the last few months I’ve been having this burning pain at the very tip of my penis when I pee. I’ve been to my local doctor and run a number of tests include a urine culture, penis swab, ureoplasma test, ultrasound of urinary tract which have all come up normal. My doctor doesn’t seem to know and has referred me to my urologist, but I’ve had some ideas about what it could possibly be. Basically all my teenage/adult life I’ve masterbated with the “prone technique” which you can read about here; http://www.healthystrokes.com/

Basically it’s just thrusting against the bed with my hands and it puts quite a lot of pressure on my penis which I’ve done all my teenage life up until now and I’m 21. Could that long term pressure from masterbating this way cause damage to my urethra thus why I am getting this burning pain? Would there be any other reason or tests that I should get done? If I had damaged it how long would it take to recover from it and how long would I need to stop mastebating for? I’ve tried stopping for a month and the burning pain didn’t completely go away but did get a little better.

Hello dear, welcome to Ask a doctor service. I read your query and here is my advice.

You have done lots of examinations including infection and sexually transmitted disease and fortunately they are negative.
I want to make sure your tests also come negative for candida (yeast) infection. You should get tested for candida because it sometimes gives bad itching and burning.
It is treated with antifungal creams.
If it comes negative the main reason for burning and itching are trauma during masturbation. The method you are using is taught and not gentle exposing you to mucosal trauma.
You may stop masturbating for a month again and make sure to wear wide underwear to help mucosal linning regenerate.
Also drink lot of water and eliminate coffee and tea to he reduce the mucosa inflammation.

Hey doctor,

Thanks for the reply what sort of tests do they do for canadia? I think I might possibly done it already and the burning comes from more inside the urethra and I feel this weird tingling feeling. So it is possible it’s been damaged is there anything else I should try other than letting it healing without aggravating it? Would drinking a lot of water and peeing more often cause irritation though or would having a lot more just make it less acidic? How long does the lining take to heal on average for long term damage is there really much an urologist can do I don’t want to do a cystsocopy as I feel that would just make matters worse and since the urine tests/ultrasound show no abnormalities or bacteria I don’t see the point right?

Hey thanks for the reply doctor, one last thing in terms of this problem is it at all possible that it’s cancer? Can burning at the urethra be a sign of it or is it unlikely? Would a cystoscopy also possibly cause further damage as well?

Thanks for that doc just one further thing I find that I have a lot smegma behind my foreskin and it’s hard to get rid of. My foreskin is kind of tight but I can still fully retract it when it’s flaccid but it’s a bit of an issue when it’s erect I can’t quite get it all the way without it being tight and hurting. I’ve only kind of discovered this in the last year or so what sort of treatments do they have for this besides circumision and is it an unsual/bad thing to have and possibly the reason for the pain when I pee?

OK, the symptoms you describe are those of a yeast infection. It also can lead to a tighten foreskin.
You can try to use clotrimazole cream or can take diflucan orally.
If the infection is cured and foreskin is still tight you can use hydrocortisone cream.

Circumcision is a good alternative.

Thanks for that doc I chased it up and had a swab done to see if it is a yeast infection. If it were would there be any foods I’d need to avoid to prevent it be aggravated? And just another thing on an unrelated topic I noticed a really hard lump on the back of my neck on the left side. It’s only on the one side though but I didn’t have it in the past and it’s not really visible or sticking out I only noticed it when I was running the back of neck, it’s not painful or on the skin but underneath.
