Question: Hello my partner and I have been having sex for almost 10 years however in the last year I have temporarily lost my hearing after finishing. It normally comes back after 2-3 minutes my wife is worried that something is wrong with me and it’s not normal. Could you give me some advice. I’m 28 if that helps.

1. This is a rare occurrence, though many people report decreased awareness of their surroundings in the resolution phase due mostly to the central higher brain connections. It seems to have developed recently from your history.

2. Ideally, investigating this problem will require at least three sets of hearing tests, before and immediately after orgasm and a delayed one later. This will be a practical difficulty given the short duration of the temporary loss of hearing (2-3 minutes). The type of audiograms and comparing them will help and you may use a software app to check your hearing on your own.

3. The possibilities include eustachian tube dysfunction altering middle ear pressures, temporary threshold shift, inner ear blood flow reduction, neck vessel blood flow problems, lipid profile, blood viscosity changes and higher brain/ temporal cortex neural signalling, minor paroxysms like migraine or blood flow changes.

4. Some of these problems have the potential of causing sudden permanent hearing loss some day. Others do not. A full investigation will require many tests, including interval audiograms, impedance audiograms (tympanometry) , neck vessel doppler studies, MRI scans of the brain and neck (cervical spine) with blood vessel studies, functional MRI / PET Scans, EEG, Blood tests and so on. Such extensive investigations will only be possible at a large regional research institute.

5. You have not mentioned any other symptoms such as tinnitus or imbalance.

6. Treatment can only be offered after all this work-up, till then it is better to be careful.
