Question: Hello, im 24 years old no prior medical conditions just low vitamin D, I weigh 165lbs, 5"9, (in the last 4-5 months I started losing weight I don't exercise regularly just bring lunch to work). I have some allergies dust mold certain trees etc. I work in Law Enforcement I normally wear a bulletproof vest all day and on my feet a lot. Since the lockdown I've been working one week on one week off. Every day for the last two or three weeks when I lay down in my bed for lets say an hour or two when I get up and relax my shoulders I feel a pressure in my shoulders that moves down to my breast area. But then after moments of stretching or if I pick my shoulders back up the pain and pressure goes away. Also if im laying in my bed on my back sometimes I will feel a slight pinch pain in my shoulder area in the pocket between the right breast and shoulder. Could this new pain be caused from my body not wearing my vest as much or possibly from lying down more than usual? Ive never felt this pain before even when I've stayed home sick for a week this has never happened before

Brief Answer:
Somatic pains

Detailed Answer:
Hi,

I went through the details you submitted. I understand you are experiencing vague pain around the shoulder region (armpit) radiating to the chest after lockdown initiative. The pain symptoms you described are not consistent with any other known organic medical conditions. Neither have I heard / read about some one developing such a kind of pain after being away from body vest.

Since the pain seems to be short lived and doesn't seem to be troubling you, I would not consider evaluating it further. However if the problem seems to worsen or if you develop persistent pain, then probably I would consider evaluating further and starting treatment.

In all, I don't think the pain symptom is related to you not wearing your west. I would however reevaluate the way you are lying down - if you are sleeping in an abnormal position or if you are sleeping on hard couch it may (to certain extent) explain the shoulder pains. But if you don't feel you are sleeping abnormally, this might just be a "somatic pain'.

Hope I am clear. I will be willing to clarify if it seems complex.

Regards

Thank you for the response Doctor. Do you think that it is possible that due to the new stress environment in lockdown that this is causing my shoulders to become tense and when i stand up the weight of pulling down from gravity is causing the pain? The pain is bothering me every day.

Brief Answer:
Stress are linked to somatic pains...

Detailed Answer:
Apologies for the late reply...

Stressors significantly contribute to the development of somatic pains. Therefore yes, the current stressful situation are more likely causing the shoulder pains and tense shoulders. But gravity are not known to influence shoulder pathology, so I would rule out gravity as a cause here.

You should stay relaxed and calm. I recommend you to sleep well, eat regularly, eat healthy, start physical exercises, practice breathing exercise, engage yourself in some activities / hobbies, try yoga or meditation during these stressful situation. Dealing the mental stress should ease the shoulder situation.

Hope this helps.
