Question: Hello doctor ,

I have been suffering from ibs and frequent clearing of throat , 2 years ago I had an acid reflux which was very quickly treated by PPI and Itopride Hydrochloride and symptoms went away.

10 days ago I was eating too much dark chocolate, fatty food, strawberries and fried foods - suddenly during 1 night I felt I had a silent reflux that didn’t go away even though I was on PPI so I visited a doctor and he put me on-
- Vonoprazan in morning instead of PPI
- mosapride before meals
- Famotidine at night

And it’s been day 4 and I have a very very very minor improvement.

Medication I live on :
PPI
Sertraline for anxiety
Panadol extra daily for migraines

What can I do ? I don’t want to undergo an endoscopy and doctor said if I didn’t get better in 10 day I will undergo an endoscopy even though all my lab and ct scans are good

Can it be cancer ?
Please help ?

Abdominal ct
Abdominal and pelvic ultrasound
Blood work
Stool fecal occult blood

They are all good
Not losing weight , no pain while swallowing no pain in chest .

Please help ?

Answer given below

Detailed Answer:
Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.

Your doctor has prescribed the right medications.
You can continue taking them.
But for complete relief from reflux, lifestyle and dietary changes are also important.

1. Weight reduction if obese
2. Avoiding smoking and alcohol drinking (If you do have these habits)
3. Avoiding coffee, citrus, chocolates, carbonated beverages, tomatoes, fatty/spicy food
4. Eating multiple smaller meals
5. Going to bed at least 2-3 hours after food.

In addition to the medications you have been taking, take Gaviscon also for symptom relief.

This does not look like cancer as you do not have red flag signs like weight loss, blood vomiting, anemia etc.
Regards,
Dr.R.K.
