Question: Hello I have on and off agin Chest pains left arm pain as well as my pinky and ring finger will go numb I had an ekg that came back normal 2 weeks ago I’m 24 dignoased with a cute pancreatitis.

I carefully went through your recent medical history. Besides the first impression of a cardiac related issue (considering the anatomy and radiation of your chest pain), other potential causes should be sort out as well.

A backbone degenerative disorder may perfectly mimic the topography of ischemic cardiac pain; this may be more likely especially if you are overweight or have experienced similar clinical symptomatology in the past.

Another cause may be ulnar nerve entrapment which needs to be carefully investigated.

Acute pancreatitis may be manifested with chest pain and not rarely, acid reflux is present too.

Although a cardiac related issue is unlikely, considering your young age, I recommend further tests especially if you have coronary risk factor (obvious heredity for CAD, dyslipidemia, you are a regular smoker, you have glucose metabolism disorders, etc.)

A provocative exercise cardiac stress test and cardiac ultrasound are advisable in this regard.

In addition to that, several tests would be beneficial to discriminate the real nature of your complaints:

- cardiac enzymes (CK-MB, Troponin)
- chest X ray study
- nerve conduction study
- backbone spine CT or MRI
- blood amylase and lipase levels
- abdominal imagine studies (MRI & CT), in case the exacerbation of pancreatitis is suspected
