Question: Hello Doctor, My daughter 7 years old was prescribed for 60000IU vitamin D3 Solution 2.5ml per month. Accidentally we gave the whole 5 ml solution instead of 2.5ml.

Is this too dangerous? Immediate doctor attention required?

Thanks for writing to us, and I understand your concern.

Depending on the clinical condition, we used to give 60000U once weakly also. Even there are times when we give 6 lakh units injection all at once.

So the dose you gave(5ml=60000U) is still with in the recommended range only, nothing to worry. Just skip coming one month dose, so that the cumulative dose will be same as your doctor's plan

Another question :
My son 4 years old, has been suffering from continous cough and cold for so many months. Doctor says that it is due to allergic food which stimulates it, so adviced to use following medication.
1.Budecart 200(puff) - 1 puff twice daily
2.Asthalin 100(puff) - as required when he coughs very frequently.

We have been using this for past 4 months but seems to be of no cure.
Can we use Budecart prolonged time?It seems to be like Steriod medicine? Is there any other alternative that we you recommend?

In order to comment on the present issue, I need few more details, to be sure of the diagnosis:

1. At what age, this issue started?
2. Is the cough present all days, or comes for few days, goes, and again comes back after a free period of few days/weeks?
3. Is the cough present throughout the days, or is it more during night and early morning? Any increase in cough in winter months?
4. Is the cough always associated with nasal discharge/fever, or present alone without these symptoms too?
5. Is there any history of allergy(asthma/eye allergy/nose allergy/eczema)in your family?
6. Is the kid having any history of eczema/nose allergy?
7. Is there any temporary relief of cough on using asthalin puff?
8. Finally, it would be helpful for me if you send a video of he using Budecort/asthalin puff, so that I can assess the correctness of technique of administration?
9. Please share any test reports/ x rays if any

Dear Sir,

Please find the answers below :

1. At what age, this issue started?
= At age of 3,now he is 4.

2. Is the cough present all days, or comes for few days, goes, and again comes back after a free period of few days/weeks?
= It occurs frequently, atleast 10-15 days in a month

3. Is the cough present throughout the days, or is it more during night and early morning? Any increase in cough in winter months?
=It occurs when he is exposed to outdoor during travel... It is present at night and early mornings.. Before 3 years of Age, he used to get this issue only during winter season and now it is always ..

4. Is the cough always associated with nasal discharge/fever, or present alone without these symptoms too?
=With out fever , nasal discharge only if he has more cold. When he coughs, we could feel the phelm in the chest (it sounds like that)

5. Is there any history of allergy(asthma/eye allergy/nose allergy/eczema)in your family?
= My wife has wizzing problem during winter season , some time use Asthalin inhaler.

6. Is the kid having any history of eczema/nose allergy?
=No

7. Is there any temporary relief of cough on using asthalin puff?
=yes, when we use Asthalin then he sleeps with out any cough

8. Finally, it would be helpful for me if you send a video of he using Budecort/asthalin puff, so that I can assess the correctness of technique of administration?

= we have took any X Ray till now

Note :

1) When we eats more chocolate or cream cakes, he coughs more the whole night.. Then we have to give 2-3 puff of Asthalin to make him sleep.

2) Due to this issue, we are not able to Vaccinate the kid with his regular shots.. Doctor advises not to Vaccinate when cough and cold is there.

Hello dear,

From the details given, it looks like allergic cough(bronchial asthma) only.
Your doctor has prescribed correct medicines(inhalers). We have to use Budesonide inhaler regularly, regardless of symptoms, and Salbutamol inhaler when needed. After a good period of control, we can think of gradual tapering down. If no improvement after 1-2 months, we will have to gradually increase the dose/combination of inhalers. Your doctor will guide in this.But, before this, we need to correct the technique of inhalation. The video i could not find/access. Alternately, your can e-mail it o customer care, tagging my name.

Meanwhile, few life style tips can help reduce symptoms:
1. Keep the house as dust free as possible- use wet mopping only for cleaning.
2. Keep him away from smoke(tobacco/other smokes)
3. Pets are better avoided
4. Avoid using woollen clothes/blankets
5. Avoid exposure to cold atmosphere.

I have seen the video, and the technique is fine!

Just ensure that seal between mask and skin intact throughout. The mask seems a bit oversized for him, but it will not be a problem if he keeps the mouth open during inhalation(to fit the mask size)
