I Want To Ask About My Father. He Is Reducing Weight Gradually With Wellness And Muscle Pain Should We Need To Be Worried?

Question: Hello Doctor,
I want to ask about my father. He is 69 Years Old.
From past 1 year we are observing his weight is reducing GRADUALLY.
From past 4 months back (in April) we went for a wedding (in a village). After having food their he fall sick with Diarrhea for 1 week. And he has become lean (quick) weight reduced to 60kgs. He had weakness, muscles pain and sometimes joints pain.
Medical History - Hight BP from past 15 Years. He takes tabs and it is in control (140/70).
2 weeks back we visited a Doctor and had a general checkup. There we got to know he is having High Blood Sugar (Fasting - 450, PPBS- around 400). He had NO Symptoms. Vitamin B12 - 79. Rest all test result were in range.
He was admitted in hospitals for 3 days, diagnosed for Blood Sugar (Now, Fasting - 85, PPBS - 170) and was given vitamin supplements via IV.
Now his medications are:
1. Amlodac-AT 1-0-1
2. Clopivas 0-1-0
3. Novostat 0-0-1
4. Ondero 1-0-0
5. Biohom in 1-0-0
6. Glycomet-GP2 1-0-1 (Before Food)
7. Prohance-D (1 scoop) 1-0-1
Diet as advised by Dietitian.
My Question:
1. He is still feeling some weakness.
2. He is getting muscle pain (Calf and thigh muscles) and knee pain from 4-5 days.
3. What could be the problem and do we need to visit the doctor again?
4. How long does he takes to get recovered completely?

Hello,
I've gone through your query but unfortunately the data you've provided are not enough to have a very good idea of what might be going on with your father. I'm going to share my thoughts about what you've got so far though.

The symptoms he's having are not very specific either. There are some things to check if he hasn't done so already. I'll explain later...

Let me start with the diagnosed disorders. Uncontrolled diabetes mellitus may indeed cause loss of body weight and weakness. Sometimes electrolyte disorders may result in muscular symptoms as well. Checking a complete blood count (for anemia), urea, creatinine, sodium, potassium, magnesium and calcium may help to provide additional clues.

Another thing to consider for someone with vague muscular symptoms is vitamin D deficiency. Measuring his 25-OH-vitamin D3 should suffice to confirm or exclude this assumption. CPK (creatine phosphokinase) is a muscle enzyme that may be elevated in various types of muscle damage (that may present with muscular complaints).

Finally a fairly common cause of these symptoms is rosuvastatin. This drug may cause all of his symptoms. You can only find out whether this is really the cause by stopping it and checking his progress. Some symptoms may take a lot of days (and even more) to improve in such cases. Remember that we can't blame the drug unless other causes have been excluded.

If any of my suggestions gets confirmed then the treatment would be fairly easy. Supplementation will correct most disorders soon enough and improvement would be demonstrated within days.

Since this is not a real visit, I cannot provide any diagnosis - only suggestions are possible. His doctor is the most appropriate person to diagnose anything. Perhaps you can talk with his doctor about the electrolytes, hematocrit, kidney function, vitamin D and rosuvastatin. Check his temperature for at least 3 consecutive days (twice a day) to exclude an infection or other disorders that may present with fever.

complete blood count (for anemia), urea, creatinine, sodium, potassium, magnesium and calcium - everything is within Range.

CPK - was slightly high (0.2 readings)

I will visit the doctor in 2 weeks as he is staying 250kms away from my place (Bangalore,India).

I see. Since he's not going to see a doctor for so long perhaps he should stop using rosuvastatin until he does. If this is the cause then it is possible that he's going to experience some improvement by the time he visits the doctor.

Novostat is the drug that contains rosuvastatin. He can stop it until he sees the doctor. He shouldn't worry about stopping it as 2 weeks time is not that long for rosuvastatin.
