Question: Hello Doctor;
I have been having weird symptoms for several years. In Dec 2010, I fainted for the 1st time. This happened like once every year, then it increased. Now it can happen at any time. I had an episode when i passed out for 6 minutes. My family said that I was breathing like a fish out of water, I was sweating, they could not find my pulse, and my heart rate was 42 when the paramedics arrived. Those syncope events are preceded by a few days of extreme exhaustion.
I have been having chest pain, shortness of breath that is increasingly worse (even when bending), dizziness, swelling. Everything I do is excarberating this condition. I had many, many tests: cardiologic, pulmonologic, and they seem to be ok. The ECG's are always showing prolonged QT. I had to have a Catheterization last year because a CT angiogram showed severe blockage on the LAD and another artery that forms a V with it. Thank God that blockage was not as it shown. Also, i had numerous Echos, about 5 in the past 1 1/2 year. The last one showed Grade 1 diastolic dysfunction.
The Dr said to monitor my BP and to exercise. My BP runs in the low side, with the HR always under 60, and I can not exercise much because of the dizziness and shortness of breath. These are getting worse, and I have chest pain with them at times. For a while, it helped when my GP doubled the water retention pill, but now, it does not make any difference.
The doctors say they don't know what is wrong with me and they don't know how to treat me.
I know it is not anxiety because I had it in the past and it is very, very different. This is health related. My family has lots of heart health and my mother died suddenly from heart. Her brother the same at 48 from an undiagnosed severe heart condition, both the same. I am not afraid, but I want to find the diagnosis so, I can receive the right treatment to feel better.

To add, I am overweight, I have been taking simvastatin and BP meds for more than 15 years. I was active before, but can not be right now. I do not smoke, drink or do drugs and never did any of those in my life.
My diet is good. Even if I eat something like some steak sometimes, my body lets me know and I do not feel well by getting swollen, drowsy, and get headaches. So, my body has trained my what to eat. I do not drink sodas, juices, except water and a coffee in the morning.

Your increasing symptoms with low pulse rate and blood pressure causing laboured breathing and unconscious... without proofs for blocks in blood suply to heart could be due to disturbed nerve suply to heart muscles stimulating their activity.
Such people with defective nerve suply to heart can be-
-Born with the problem to have increasing symptoms over the time ....called to have left or right bundle branch locks.
-Few people get damage of heart muscles because of existing heart disease... to cause disturbed nerve suply and symptoms same as you have.
* In this scenario pace maker implantation is very good way to improve the condition...along with management of excessive weight and blood pressure also management of any other heart disease.
**-An expert cardiac surgeon would help you out in confirming the diagnosis as well as it's specific management.

Mostly detailed medical history,a careful heart examination by an expert and ECG..which represents electric impulses travelling to heart muscles ... forms excellent gadget to confirm heart block..judged from increased qrs part of electrical impulse wave. getting prolonged due blockage in path of impulse.Actually your delayed qrst intervle hints possibility of blocks in heart nerve path.
Apart from this, investigations to know the cause of heart block(congenital or aquired-heart failure,infection or valvular diseases)are undertaken to determine treatment plan
The fact that you had been getting symptoms like fatigue since childhood... indicates possibility of either congenital heart nerve block OR...some form of other heart condition... which had lead heart weak &caused aquired-heart nerve block.
I feel that should satisfy you. Any follow-up query is welcome.
