Question: I fell a month ago and have a sharp pain under my breast

Its less than likely that the trauma caused a month ago will flare up now.If you have pain beneath your breast,the first thing to rule out is repeat trauma at the previous site which may have caused the pain.Get a chest xray to rule out possible fracture/dislocation/hair line fracture. Next if the chest xray is clear and bony trauma is rule out, then viceral pain needs to be ruled out.Now making the diagnosis isnt easy without examination.You could have muscular pain or visceral pain.A chest examination will differentiate here. Costochondritis also should be ruled out.Meanwhile apply dry hot fomentation and keep it wrapped in a tight bandage. You can take an antispasmodic medication.
