Question: Five weeks ago I discovered a sore on my foreskin. Two weeks later I went for a blood test with the following result: T.pallidum -serum: Positive and RPR (vdrl): Negative. I was diagnosed with primary syphilis. The doctor prescribed a 14 day Doxycycline prescription (100mg twice a day). A week after the treatment i did a blood test again. The blood results is the same as the firts one: T.pallidum serum : Positive and RPR (vdrl) : Negative. My questions: Do I still have Syphilis? Was the treatment successfull? Is it safe to have sex again?

Brief Answer:
If you had the full treatment, very likely the infection was fully treated

Detailed Answer:
Hello and welcome to 'Ask a Doctor' service,

I carefully read your query, reviewed the attached reports and understand your concern.

T. Pallidum serologic tests may indicate present or previous infection. So, it is expectable that the repeated tests show it to be positive as they are positive even after successful treatment.
I would say that because you have completed a very effective treatment for syphilis, you should not worry about the treatment not being successive.

The only thing you have to be watchful is that syphilis is sexually transmitted and your partners may have syphilis so they need to be checked and treated as well. Otherwise, there is a risk you will be reinfected.

Also, you may need to do tests for other sexually transmitted diseases as well as sexually transmitted diseases go hand in hand together. I would recommend you to do the following tests: HIV, hepatitis B and possibly tests for chlamydia and gonorrhea.

To conclude:

- A positive treponema pallidum test does not necessarily indicate a failure of treatment. It may indicate a previous but treated infection.

- If you took the treatment as you should then there is no reason to worry

- Your partners need to be checked and treated, otherwise, you are at risk of reinfection

- Other sexually transmitted diseases have to be ruled out so some more tests may be needed (explained above).

I hope this is helpful.
I remain at your disposal for further medical assistance.
