Brief Answer:
Likely related to the alcoholism.

Detailed Answer:
I think it will be hard to sort out but until his alcoholism is under control (and as you know, a bottle or two a day is not under control).

Excessive drinking plus alcohol related dementia can cause urinary incontinence.

Did the size of his penis shrink after starting the bladder pill? Has he gained weight? Sometimes obesity can somewhat swallow up the penis.

I think it's very important for him to get into rehab and then the other problems can be sorted out better.

You may also want to take him for a consult with a urologist. Some medications for bladder problems can lower certain hormones that may be making his penis and testicles smaller. A urologist can also help with the urination problems.
But again - this probably is not going to get better until he is no longer drinking.

I hope this information helps. I am sorry you are having these troubles at home.
