Sildenafil (Viagra) is a part of the phospho-diesteraze 5 inhibitors. This is an enzyme that acts upon an important enzyme of the arterial smooth muscle relaxation-contraction cycle. This enzyme is cGMP. By inhibiting the PDE5, the cGMP hydrolization is also inhibited or prolonged, causing a larger period of arterial smooth muscle to be in relaxed state, hence the beneficial effect in men with erectile dysfunction. This is also true for the systemic arterial circulation, and PDE5-inhibitors cause a relaxation of the arterial smooth muscle overall, causing arteries to longer maintain the relaxed state, causing widening of them (vasodilatation), causing blood pressure to drop some figures. Special precaution should me undertaken in patients who suffer from coronary artery disease and the treatment with potent vasodilators (such as nitrate derivatives) is used. The combination of nitrate derivatives and PDE5-inhibitors (sildenafil) can lead to dangerous, sometimes lethal, drops in blood pressure.
