Question: Hi -
I just finished a course of antibiotics for sure nusitis and now I have a bad cold 49virus attacking my system again (I’m 49 yr old woman with sensitive airways). I’m on pulmicort, Flonase, Finishing up another day of Biaxin, and I took lower dosage of muconex and antihistamine. For some reason my heart is beating fast and I have difficulty sleeping. I feel warm and cold. Same symptoms as my son who also has a fever too in past three days. My heart rate ranges from 85-96 bpm and I feel restless. I have a bad cough - not very productive but with a lot of post nasal drip. My son was swabbed in the first day he was sick and it wasn’t the flu. Any advice appreciated. I don’t want to end up in er but I’m having trouble sleeping. Is it a big deal? I’ve been suffering from sickness for nearly three weeks.

Brief Answer:

Need Investigations & expert advise

Detailed Answer:

Hi,
YOur son may have just a common cold ( may not be viral ). Still viruseso d attack imuno compromised people easily.

I feel you had viral infection to start with or independent allergy aggravation .. it progressed to sinus involvement & Eustachian tubal block.

This caused postnasal drip. Your sensitive airways allowed the symptoms to travel in bronchi/ lungs leading to cough.

Secondary infection by bacteria is a common thing to follow, making the condition long standing & trouble-sum.

You have not mentioned about any breathing difficulty (bronchitis/ asthma) which is a common symptom causing severe cough & disturbed sleep too.

You already had a course of antibiotic containing Clarithromycin, still having no obvious relief.

You already are having medicines for allergy control & cough( Muconex, antihistaminic pulmicort & flonase ).

Allergy causes lot of trouble when exposed to allergen & exposure to infection enhances the picture.

Involving all the respiratory passage including ear, nose, throat& ear & bronchial tree too.

In this scenario I would advise for a through check up & investigations for allergy screening & respiratory system, culture of sputum for pin pointing the causative organism & suitable antibiotic to control it, X ray chest if found needful.

* You need consultation of a allergy specialist / chest physician / ENT surgeon

-- Desensitization treatment to get rid of allergy, Histamine blocker drugs are few things which aid to maintain good health & normal life.

* Management- self care-
- Avoid the allergens found/ triggering agents observed/ Be aware of dusts, fumes or smoke.
- Have healthy & permissible food with vitamins( antioxidants), minerals & proteins / respiratory exercises & walking regularly,
-A stress less life, sound sleep
- Strictly following medical advise & taking medicines regularly..
-.Take the full advised course of antibiotics.

Hope I have answered your query. Let me know if I can assist you further.

Hi,
I had a rough night with three hours sleep from cough. Would you recommend an antihistamine for the bad post nasal drip or to let it flow? It is gagging me at night. Also I wonder if I should ask my doc to extend the antibiotic until this illness is over. I’ve been on biaxin for nearly ten days but just got this cold virus a few days ago.

Brief Answer:

Control of infection, allergy , care of mucus membranes & body resistance

Detailed Answer:

Hi,

Post nasal drip is dribbling of excessive mucus secreted by throat, nose , sinuses, respiratory passage specially more in night due to lying down position.

So too control it four pronged management is needed (though I had tried to explain it in previous answer in detail... let me try to narrate it in a simpler way.)

1. - Control of infection- As you had already a 10 days long dose of antibiotic recently, you are already taking anti allergic medicine, nasal spray & inhalation for allergy control... it can be assumed that the infection may not be totally gone-

- That's why I had suggested for sputum culture & sensitivity test to confirm presence of lingering infection ( which could be due to bacterial resistance to the antibiotic taken ) & also the effective antibiotic to control the bacteria.

If you visit your doctor/ a chest physician, he will examine you & may advise for this... or you can casually inquire about the need of such test.

2. Allergy control- Mostly non sedative anti allergic like Loratadine is usually prefer ed for such complaints. ( again needs to be decided by the doctor who has examined you physically).

As I had mentioned earlier.. Tab. Histamin blocker medicine (like Montelukast ) would help in allergy control with fast effect & allergy testing with desensitization treatment would help in long run.

3. Dryness of mucus membrane along with allery & infection irritate secretion of excess mucus to combat this-

-Saline nasal sprays ( Nasivion drops )/ Saline irrigation of nasal passage using nasal pot- would moisten nasal passage, remove extra mucus bacteria & allergens or other irritating factors to relieve irritation & excessive secretion.

- You can put on humidifiers to increase humidity in the room.
- Inhalation of vapor would add in relieving inflammation of sinuses.
- saline gargles would help to relieve sore throat.

4. In addition along with the medication, prevention of exposure to infection & allergy & self care, building up resistance (as described earlier) helps in combating repeated attacks of cold.

This way a patient care & follow up to expert would definitely help you.

Hope I have answered your query. Let me know if I can assist you further.

Thank you - I went to urgent care with fever of nearly 102, aches, bad cough and was placed on an antiviral. Not much appetite and really tired. I was gicen chest X-ray which showed inflammation around edges of bronchial area. I was also given prednisone & doxycycline. I’m hoping to restore my immunity soon.
