Question: Dear doctor, my mom had several times of stomach ache, from endoscopy clean and colonoscopy there were some polip but its stated that the polip is inflamation polip desendan colon rectum...the problem is tes faecal calprotectin is up and down sharply : 153.8, 8.6, 341.1, 26.7, 197.8, 50.2 and 414.7. M2pk test is normal. From colonoscopy my mother also has hemorroid My mother also has sjogren My mother is on medication: myfortic, budenofalk and salofalk. Please suggest... Also the up level of calprotectin can be cause by hemorrhoids?

Have gone through your details,
Inflammation any where along the lining of gut would cause elevated levels of serum calprotectin.
Calprotectin is infact a marker of inflammation.
This rapid rise is due to polyp.Was a biopsy sample taken is the polyp benign or malignant?
Why was it surgically not taken out?
As the polyp size is increasing so does the inflammation therefore serum calprotectin is done.
First suggestion is biopsy of polyp should be done to see if they are non cancerous or cancerous.Usually multiple and polyp larger then 10 mm are cancerous.

Secondly hemorrhoids have nothing to do with calprotectin levels.

As per explained before the polip already taken out and it is benign.. The polip taken out by colonoscopy... And the up and down calprotectin also happening before and after that.

sorry I though the polyp is still there.
ok,Faecal calprotectin is a protein released by the white blood cells, neutrophils(defence cells), found in inflamed areas of the bowel in Inflammatory bowel disease specifically Crohn's disease and ulcerative coilitis. Its a screen test as every patient cannot be taken for colonoscopy.
So main purpose of FC in stool samples is to help distinguish Inflammatory Bowel Disease from other conditions(while screening patients).
So as said above,
Fecal Calprotectin is just an screening test and if polyp has already been removed then the only significance it has in your case is that there could be residual inflammation in intestines.
Give her some anti inflammatory medications with antacids eg ibuprofen and Prilosec twice daily for 1week and get the level checked up again.
Once the residual inflammation is gone levels would return back to normal.

I can very well understand your plight right now because as a non medical person understanding things are somewhat difficult.
But polyp is some thing completely different than from inflammatory bowel disease.IBD is a permanent condition of intestine and requires almost lifelong treatment.Its not curable but symptoms are almost controllable.
The treatment she is having is the standard one.Calprotectin would always fluctuate in a patient with IBS.
See our body has a defence/Immune system which contains neutrophils.In IBD these neutrophils numbers increases greatly in intestinal cells.They are the neutrophils which secrete large amount of Calprotectin.As neutrophils are first line defence system therefore immunosuppressant is given other wise the body immunity would harm itself.Secondly as there is permanent inflammation an anti inflammatory is given.

Yes off course these medicines have side effects but without then things would start worsening.
Only thing I would suggest is to give her an antacid along with these medicines.They are all strong medicines and cause gastritis a condition in which there is inflammation in walls of stomach and small intestine causing pains.
I would suggest you to add over the counter Omeprazole twice daily.
Aches would be there as I said condition is controllable but not curable.Dont stop any medicine on your own.
Add the over the counter antacid I suggested.
Don't panic.
Things would get better.I assure you.
