Can The Insulin Dosage Be Corrected Through Some Means After Accidentally Missing A Dosage Of Lantus?
Question: We forgot to given my father his lantus last night. he takes 40 units every pm. He is not on a regular insulin sliding scale. His blood sugar is 154 this am. He usually runs in the 200's to 300's. He just got out of rehab for a stroke and we haven't found for a PCP for him yet.

The dose of basal insulin you mentioned is high and usually missing a dose will be manifested with high glucose level.
In these cases, when fasting glucose level is higher with insulin than without, a low glucose level at 2.00 a.m ( caused by high insulin dose) should be excluded.
When hypoglycemia happens during the night, the fasting glucose level is high and in these cases the dose of insulin should be decreased.
I would recommend to apply 20Ui of lantus tonight and check glucose level.at 2.0 a.m.
If is high ( over 180g/ dl) increase the dose.
